Managing Recurring Candida Infections on the Penis: Risks and Alternatives - Dermatology

White Candida on the genitals

Hello Dr.
Wu,
For the past few years, I have been experiencing itching on the glans, accompanied by a lot of white flakes.
I previously consulted a physician who diagnosed me with candidiasis and prescribed Mycomb Cream.
The results were quite good.
However, after a few months of symptom relief, the issue tends to recur.
When it does, using Mycomb Cream for a few days usually helps.
This has been going on intermittently for about two to three years.

My concern is that I found out Mycomb Cream contains steroids, albeit in low doses.
I would like to ask if long-term application on the glans could have any adverse effects.
Additionally, could you recommend any alternative creams with fewer side effects? Thank you!

A Jie, 30~39 year old female. Ask Date: 2019/07/05

Dr. Wu Yuxin reply Dermatology

You should consult your attending physician about this issue, as they are most familiar with your condition and can help you choose the appropriate medication.

Managing recurring Candida infections on the penis can be a challenging issue, especially when it involves the use of topical treatments like MYCOMB cream, which contains a corticosteroid. It's important to understand both the risks associated with long-term corticosteroid use and the alternatives available for treating Candida infections.

Corticosteroids, even in low doses, can have several side effects when used over extended periods, particularly on sensitive areas such as the genital region. Prolonged use can lead to skin thinning (atrophy), increased susceptibility to infections, and potential systemic absorption, which could affect other parts of the body. In the case of the penis, chronic use of topical steroids can also disrupt the natural balance of skin flora, potentially exacerbating fungal infections like Candida.

Candida infections, or yeast infections, are caused by an overgrowth of the Candida fungus, which is normally present in small amounts on the skin and mucous membranes. Factors that can contribute to recurrent infections include poor hygiene, excessive moisture, tight clothing, diabetes, and a weakened immune system. It's crucial to address these underlying factors to reduce the likelihood of recurrence.

In terms of alternative treatments, there are several options to consider:
1. Topical Antifungals: Instead of corticosteroids, consider using over-the-counter antifungal creams such as clotrimazole or miconazole. These medications target the fungal infection directly without the side effects associated with steroids.

2. Oral Antifungals: If topical treatments are not effective, a healthcare provider may prescribe oral antifungal medications like fluconazole. This can be particularly useful for more severe or persistent infections.

3. Lifestyle Modifications: Improving hygiene practices can significantly reduce the risk of recurrence. Ensure that the genital area is kept clean and dry, wear loose-fitting cotton underwear, and avoid irritants such as scented soaps or lotions.

4. Dietary Considerations: Some studies suggest that reducing sugar intake may help manage Candida overgrowth, as yeast thrives on sugar. Incorporating probiotics into your diet, either through supplements or fermented foods, may also help restore the natural balance of flora.

5. Consultation with a Specialist: If recurrent infections persist despite treatment, it may be beneficial to consult a dermatologist or urologist who specializes in genital health. They can provide a more tailored approach to your condition and explore other potential underlying causes.

In conclusion, while MYCOMB cream may provide temporary relief, its long-term use on the penis carries risks that should not be overlooked. Exploring alternative antifungal treatments, addressing lifestyle factors, and consulting with a healthcare professional can help manage recurring Candida infections more effectively and safely. Always remember to follow up with your healthcare provider to monitor your condition and adjust treatment as necessary.
