Root canal treatment issues?

I would like to ask about root canal treatment.
The root canal has been filled, but my mouth has not closed for a long time.
When it finally closes, I experience pain, and there is soreness when pressure is applied.
Could this be due to incomplete cleaning, or is it an infection? Thank you.

A Tian, 20~29 year old female. Ask Date: 2019/07/09

Dear Brother A-Tian,
Hello! After root canal treatment, it is not uncommon to experience some abnormal sensations.
If there is no redness or swelling, it should improve within a week.
However, if discomfort persists, I recommend consulting an endodontist for further evaluation.
Take care! Wishing you peace and smooth recovery!
Sincerely,
Dr.

Experiencing pain after a root canal treatment can be a distressing situation, and understanding the underlying causes is crucial for effective management. After a root canal procedure, patients may encounter various types of discomfort, which can stem from several factors related to the treatment itself or the condition of the tooth prior to the procedure.

Causes of Pain After Root Canal Treatment
1. Incomplete Cleaning of the Root Canals: One of the primary reasons for persistent pain after a root canal is the incomplete cleaning of the root canals. The root canal system can be complex, with multiple canals and branches that may not be visible on standard X-rays. If any part of the canal system is left untreated, it can harbor bacteria, leading to infection and inflammation, which may cause pain.

2. Post-Operative Inflammation: After a root canal, it is common for the surrounding tissues to become inflamed. This inflammation can result from the body’s natural healing response or from the irritation caused during the procedure. This can manifest as tenderness or pain when pressure is applied to the tooth.

3. Infection: If bacteria were present in the tooth before the procedure, they could have spread to the surrounding tissues, leading to an infection. This can occur if the tooth was not adequately disinfected during the root canal treatment. Symptoms of infection may include swelling, increased pain, and sometimes fever.

4. Pressure from Bite Misalignment: If the bite is not properly adjusted after the root canal treatment, it can lead to discomfort when biting down. This misalignment can cause the treated tooth to be subjected to excessive forces, resulting in pain.

5. Nerve Damage or Sensitivity: Although the nerve of the tooth is removed during a root canal, the surrounding tissues and nerves can still be sensitive. Some patients may experience heightened sensitivity or pain in the area for weeks or even months after the procedure.

Solutions and Management
1. Follow-Up with Your Dentist: It is essential to communicate with your dentist about the pain you are experiencing. They may recommend a follow-up appointment to assess the tooth and determine if further treatment is necessary, such as retreatment of the root canal or additional imaging to identify any underlying issues.

2. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage discomfort. However, it is crucial to follow the recommended dosages and consult with your dentist if the pain persists.

3. Antibiotics: If an infection is suspected, your dentist may prescribe antibiotics to help eliminate the bacteria causing the infection. This can help reduce pain and inflammation.

4. Adjusting the Bite: If the pain is related to bite misalignment, your dentist can make adjustments to ensure that the treated tooth aligns properly with the opposing teeth, alleviating pressure and discomfort.

5. Consider Advanced Imaging: If pain continues despite treatment, your dentist may recommend advanced imaging techniques, such as a cone beam CT scan, to get a clearer view of the root canal system and surrounding structures.

6. Possible Retreatment: In cases where the initial root canal treatment was not successful, retreatment may be necessary. This involves reopening the tooth, cleaning the canals again, and sealing them properly to eliminate any remaining infection.

Conclusion
Persistent pain after a root canal treatment can be attributed to various factors, including incomplete cleaning, inflammation, infection, bite misalignment, or nerve sensitivity. It is crucial to maintain open communication with your dentist and seek further evaluation if pain persists. Proper diagnosis and management are key to resolving discomfort and ensuring the long-term success of the root canal treatment. If you are experiencing significant pain, do not hesitate to reach out to your dental professional for guidance and support.
