Probability of Pregnancy
Hello, doctor.
My menstrual cycle is regular, approximately 27 days.
My last periods were from May 31 to June 7 and from June 26 to July 2, with the next expected period on July 23.
Yesterday, on July 15, my boyfriend touched his penis with his left hand, which had come into contact with pre-ejaculate fluid.
Later, he applied some lubricant and inserted his right finger into my vagina (with some depth).
Upon reflection, he is unsure if his right hand had any pre-ejaculate fluid on it at that time.
I would like to ask, doctor, if there is a significant chance of pregnancy if pre-ejaculate fluid was on his finger and it was inserted into my vagina eight days before my expected period? Should I take emergency contraception? How long do sperm survive after exposure to air? We only engaged in foreplay and did not have sexual intercourse.
If I want to use a pregnancy test, can I test a week later (on July 22)? Thank you, doctor.

Hello, Miss Anonymous: If your menstrual cycle is as regular as you mentioned, around 27 to 28 days in May, June, and July, then the chances of conception eight days before your period are quite low, as ovulation would have already occurred.
As for sperm survival, it generally lasts about two to three days.
Typically, pregnancy tests should be taken after a missed period; for example, if you expect your period on July 23, it would be meaningful to test after that date.

Understanding the chances of conception from pre-ejaculate fluid is a common concern for many individuals. In your situation, it’s important to clarify a few key points regarding the nature of pre-ejaculate, the timing of your menstrual cycle, and the likelihood of pregnancy.
Pre-ejaculate fluid, also known as pre-cum, is produced by the Cowper's glands and can contain sperm. While the primary function of this fluid is to lubricate the urethra and neutralize acidity, it can occasionally carry sperm that may have been left in the urethra from a previous ejaculation. However, the concentration of sperm in pre-ejaculate is generally lower than in ejaculate, which means the chances of conception from this fluid alone are reduced, but not eliminated.
Given your menstrual cycle is approximately 27 days, ovulation typically occurs around 13 to 14 days before your next expected period. In your case, with your next period expected on July 23, ovulation would likely have occurred around July 9 to July 10. Engaging in sexual activity on July 15, which is just a few days after ovulation, places you within a window where conception is possible, especially if sperm were present in the pre-ejaculate.
Regarding your specific question about the likelihood of pregnancy from your described scenario, if your boyfriend's fingers had pre-ejaculate on them and then inserted into your vagina, there is a possibility of sperm being introduced into the vaginal canal. However, the actual risk of pregnancy from this type of contact is generally considered low, especially if there was no direct ejaculation.
As for the question of whether you should take emergency contraception (the "morning-after pill"), it is typically recommended if there is a concern about potential pregnancy from unprotected intercourse or if there is a significant risk of sperm being present. Since you are within a week of your expected period and considering the nature of the contact, if you feel anxious about the possibility of pregnancy, taking emergency contraception could be a prudent choice. It is most effective when taken as soon as possible after the incident.
Regarding the survival of sperm outside the body, sperm can live for a short time when exposed to air, typically only a few minutes. However, in a moist environment, such as inside the vagina, sperm can survive for several days, which is why timing in relation to ovulation is crucial.
If you are considering taking a pregnancy test, it is advisable to wait until at least a week after the expected date of your period for the most accurate results. In your case, testing on July 22 would be appropriate, as it would allow enough time for the hormone hCG to build up in your system if you are pregnant.
In summary, while there is a possibility of conception from the scenario you described, the risk is relatively low. If you have concerns about potential pregnancy, consider taking emergency contraception and plan to take a pregnancy test after your expected period date for the most reliable results. If you have further questions or concerns, consulting with a healthcare provider would be beneficial for personalized advice and support.
