Gonococcal pharyngitis
Hello doctor, can gonococcal pharyngitis be detected through urine or blood tests? I've been on medication for three weeks, and although I feel much better than at the beginning, I still experience a dry cough when I take deep breaths or talk.
Thank you.

Hello: You can diagnose through oral swab bacterial culture or urine bacterial culture, and it is recommended to continue follow-up treatment with the hospital's Infectious Disease Department.
Wishing you safety and health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the ENT Department, Taoyuan Hospital, Ministry of Health and Welfare.

More Info
Gonococcal pharyngitis, an infection caused by the bacterium Neisseria gonorrhoeae, primarily affects the throat and is often associated with sexually transmitted infections (STIs). When it comes to diagnosing this condition, the most reliable methods involve direct testing of the infected area rather than relying solely on urine or blood tests.
Detection Methods
1. Throat Swab: The gold standard for diagnosing gonococcal pharyngitis is a throat swab. This involves taking a sample from the back of the throat and testing it for the presence of Neisseria gonorrhoeae. This method is highly specific and sensitive, making it the preferred choice for accurate diagnosis.
2. Urine Tests: While urine tests can be used to detect gonorrhea, they are primarily effective for diagnosing urogenital infections (i.e., infections of the urethra). Urine tests may not be as reliable for detecting gonococcal pharyngitis because the bacteria may not be present in the urine if the throat is the primary site of infection. However, some nucleic acid amplification tests (NAATs) can detect gonorrhea in urine, but they are not specifically designed for throat infections.
3. Blood Tests: Blood tests are generally not used to diagnose gonococcal pharyngitis. While they can detect antibodies to various infections, they do not provide a direct indication of an active gonococcal infection in the throat. Blood tests may be more relevant in cases of disseminated gonococcal infection, which is a more severe and systemic form of the disease.
Symptoms and Follow-Up
You mentioned experiencing a dry cough when breathing deeply or speaking, which could be a residual symptom following treatment for gonococcal pharyngitis. It is not uncommon for patients to have lingering symptoms after an infection, especially if the throat was significantly affected. However, if the cough persists or worsens, or if you experience other symptoms such as fever, difficulty swallowing, or worsening throat pain, it is crucial to follow up with your healthcare provider.
Treatment Considerations
If you have been treated for gonococcal pharyngitis and are still experiencing symptoms, it may be worth discussing with your doctor the possibility of a follow-up throat swab to ensure that the infection has been fully resolved. Additionally, if you were treated with antibiotics, it is important to complete the full course as prescribed, even if symptoms improve.
Conclusion
In summary, while urine tests can detect gonorrhea, they are not the most effective method for diagnosing gonococcal pharyngitis. A throat swab remains the most reliable diagnostic tool. If you have concerns about your symptoms or the effectiveness of your treatment, it is advisable to consult with your healthcare provider for further evaluation and potential retesting. They can provide personalized advice based on your medical history and current condition.
