Gonococcal pharyngitis

Hello Doctor, I am #148113.
Last week, the infectious disease doctor performed a urine culture and blood tests, and he said the results were fine, but he did not conduct an oral culture.
Can we be certain that there is no gonococcal pharyngitis? Do purulent secretions from the mouth look similar to phlegm? I only cough when taking deep breaths and speaking, and occasionally have phlegm, so the infectious disease doctor prescribed only cough syrup.
I hope to recover soon.
Thank you for your response.

Lin Mu Mu, 20~29 year old female. Ask Date: 2019/07/18

Dr. Zheng Jueyi reply Otolaryngology

Hello, if there are no obvious issues found during the examination, pharyngitis should gradually improve.
It is recommended to follow the treatment provided by the infectious disease specialist.
Wishing you peace and good health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Gonococcal pharyngitis is an infection of the throat caused by the bacterium Neisseria gonorrhoeae, which is the same organism responsible for gonorrhea, a sexually transmitted infection (STI). Understanding the diagnosis and symptoms of gonococcal pharyngitis is crucial for effective management and treatment.

Symptoms of Gonococcal Pharyngitis
The symptoms of gonococcal pharyngitis can often be mild or even absent, which can lead to underdiagnosis. When symptoms do occur, they may include:
1. Sore Throat: Patients often report a painful or scratchy throat.

2. Difficulty Swallowing: Pain may increase when swallowing, leading to discomfort.

3. Swollen Lymph Nodes: Lymph nodes in the neck may become enlarged and tender.

4. Pus-like Discharge: Some individuals may notice purulent (pus-like) discharge from the throat.

5. Fever: A mild fever may accompany the infection.

6. Cough: A persistent cough may develop, particularly if the infection spreads or irritates the throat.

Diagnosis of Gonococcal Pharyngitis
Diagnosis typically involves a combination of clinical evaluation and laboratory testing. While urine cultures and blood tests can help rule out other infections, they do not specifically confirm or exclude gonococcal pharyngitis. The gold standard for diagnosis is a throat culture or a nucleic acid amplification test (NAAT) specifically targeting Neisseria gonorrhoeae.

In your case, since the infectious disease doctor conducted urine and blood tests and found no issues, it suggests that you may not have a systemic gonococcal infection. However, without a throat culture, it is not possible to definitively rule out gonococcal pharyngitis. The absence of a throat culture means that the specific diagnosis of gonococcal pharyngitis has not been confirmed.

Understanding Mucus and Sputum
Regarding your question about the appearance of purulent secretions in the throat, it is important to note that purulent discharge can vary in appearance. Mucus from the throat can resemble sputum, which is the mucus that is coughed up from the respiratory tract. Both can be thick and may appear yellow or green if there is an infection. However, the consistency and color can vary based on the underlying cause of the symptoms.

Management and Follow-up
Given that you are experiencing a cough and occasional phlegm, it is essential to monitor your symptoms. The cough may be a result of throat irritation or post-nasal drip, which can occur with infections. The cough syrup prescribed by your doctor may help alleviate the symptoms, but if your symptoms persist or worsen, further evaluation may be necessary.

If you continue to experience throat discomfort, persistent cough, or any new symptoms, it would be advisable to follow up with your healthcare provider. They may recommend a throat culture or further testing to ensure that any potential infection is appropriately diagnosed and treated.

In summary, while your urine and blood tests are reassuring, the lack of a throat culture means that gonococcal pharyngitis cannot be definitively ruled out. Monitoring your symptoms and maintaining open communication with your healthcare provider will be key in managing your health effectively.
