Discomfort in the armpit?
I am a male and have been experiencing a tight sensation from my armpit to the upper chest area, which sometimes feels mildly painful when pressed.
I previously had a sebaceous cyst that was uncomfortable, but it eventually went away.
I visited both a clinic and a dermatology department at a major hospital, and they both said there was no issue and that surgery was not necessary.
Recently, the discomfort has increased significantly.
I have tried massage therapy, but it doesn't seem to be related to muscle or joint pain.
I am wondering if it could be related to lymph nodes or if something else has developed.
Could you please recommend which specialty I should consult and what tests I should undergo? Thank you.

Dear Mr.
Achang: 1.
It is generally correct to see a dermatologist; however, if you still have concerns, especially regarding the lymph nodes in the breast and armpit, you may want to consult a general surgeon or a breast surgeon for a second opinion.
2.
Perhaps you could first discuss and examine your concerns with your primary care physician before making a decision.

More Info
Understanding discomfort in the armpit area can be concerning, especially when accompanied by sensations of tightness or pain. Given your description, it’s important to consider various potential causes and the appropriate steps for diagnosis and treatment.
Possible Causes of Discomfort in the Armpit
1. Lymphadenopathy: One of the primary concerns in the armpit area is swollen lymph nodes. The lymph nodes can become enlarged due to infections, inflammation, or other underlying conditions. If you are experiencing discomfort and have a history of skin issues, it’s possible that the lymph nodes in that area could be reacting to an infection or other stimuli.
2. Cysts or Lipomas: You mentioned having a sebaceous cyst (粉瘤) in the past. While these can sometimes resolve on their own, they can also become inflamed or infected, leading to discomfort. Lipomas, which are benign fatty tumors, can also develop in this area and may cause a sensation of pressure or discomfort.
3. Muscle Strain or Nerve Issues: Discomfort in the armpit can sometimes be attributed to muscle strain or nerve compression. This could occur from activities that involve repetitive arm movements or poor posture.
4. Skin Conditions: Conditions such as dermatitis, fungal infections, or other skin irritations can also lead to discomfort in the armpit area.
5. Other Medical Conditions: Rarely, discomfort in the armpit can be associated with more serious conditions, including infections or malignancies.
Recommended Steps for Diagnosis
Given the complexity of your symptoms, it is advisable to consult a healthcare professional who can perform a thorough evaluation. Here are some recommendations on which specialists to see and what tests might be appropriate:
1. Primary Care Physician (PCP): Start with your primary care physician. They can perform an initial assessment, including a physical examination, and take a detailed medical history. They may check for swollen lymph nodes, tenderness, or any visible abnormalities.
2. Dermatologist: Since you have a history of skin issues, a dermatologist can evaluate the skin in the armpit area for any signs of infection, cysts, or other dermatological conditions. They may perform a biopsy if necessary.
3. General Surgeon: If there is a concern about a cyst or lump that may need to be removed or further evaluated, a general surgeon can provide insight and potentially perform a procedure if warranted.
4. Oncologist: If there is any suspicion of malignancy, your PCP may refer you to an oncologist for further evaluation and management.
Diagnostic Tests
- Ultrasound: This imaging test can help visualize any abnormalities in the lymph nodes or soft tissues in the armpit area.
- Blood Tests: These can help identify signs of infection or inflammation.
- Biopsy: If there is a suspicious lump or cyst, a biopsy may be performed to determine its nature.
Conclusion
It’s essential to address your discomfort proactively. While it may not be serious, persistent symptoms warrant a thorough evaluation to rule out any underlying conditions. Make an appointment with your primary care physician to discuss your symptoms in detail, and they can guide you on the next steps, including referrals to specialists as needed. Remember, early diagnosis and treatment can lead to better outcomes, so don’t hesitate to seek help.
