How to perform a health check for breast implants?
Due to congenital developmental issues, I underwent saline breast implant surgery 20 years ago.
Now, in my 40s, I want to get a free health check-up, but I hesitate because one of the tests is a mammogram.
I would like to ask the doctor if, during the government-sponsored free health check-up, I can inform the medical staff that I do not wish to undergo this mammogram?

Dr. Chen Jiaming reply Surgery
The "free adult health check" program provided by the National Health Service includes screenings for individuals aged 40-64 every three years, but it does not include mammography.
Some hospitals may combine "free cancer screening mammography" with this program; however, mammograms are generally offered every two years starting at age 45 (or at age 40 if there is a family history of breast cancer within two degrees of relation).
In the past, individuals with saline breast implants were still able to undergo mammography (as long as there is normal breast and axillary tissue present), but the examination process requires compression of the breast tissue.
It is important to inform the technician in advance to avoid excessive pressure that could damage the saline implant.
As for ultrasound examinations, they typically cannot penetrate the deeper tissue behind the saline implant.
Some individuals concerned about breast diseases may opt to pay out-of-pocket for an MRI, although this can be quite expensive.
If you wish to decline any examination, you can communicate your concerns to the personnel in advance or at the time of the procedure.
If you insist, they will usually respect your wishes to avoid any potential issues.

When it comes to mammograms, especially for individuals with breast implants, it is essential to understand the importance of regular screenings and the implications of skipping them. Mammograms are a critical tool in the early detection of breast cancer, and while having breast implants may raise concerns about the procedure, it does not exempt individuals from the necessity of these screenings.
Firstly, it is important to note that breast implants, whether saline or silicone, do not prevent the development of breast cancer. In fact, women with implants are still at risk for breast cancer, and regular mammograms are crucial for monitoring breast health. The presence of implants can sometimes complicate the imaging process, but radiologists are trained to interpret mammograms in patients with implants. They may use specialized techniques to ensure that the breast tissue is adequately visualized.
Regarding the question of whether you can skip the mammogram during free health checks, it is generally not advisable to opt out of this vital screening. If you have concerns about the mammogram due to your implants, it is best to communicate these concerns with the healthcare providers conducting the screening. You can inform them about your implants, and they can take the necessary precautions to ensure that the mammogram is performed safely and effectively.
In many cases, healthcare providers will encourage you to proceed with the mammogram, as they can adjust the imaging technique to accommodate the presence of implants. For instance, they may use additional views or specific positioning to ensure that the breast tissue is adequately assessed. Moreover, if you have any specific concerns or discomfort regarding the procedure, discussing these with the medical staff can help alleviate anxiety and provide you with the necessary information to make an informed decision.
If you still feel uncomfortable about undergoing a mammogram, you may want to consider alternative screening methods, such as breast ultrasound or MRI, which can also be effective in certain situations. However, these alternatives may not replace the need for a mammogram entirely, especially if you are over 40, as recommended by many health organizations.
In conclusion, while it is understandable to have concerns about mammograms with breast implants, it is crucial not to skip this important screening. Regular mammograms are essential for early detection of breast cancer, and healthcare providers can accommodate your needs during the procedure. Open communication with your healthcare team is key to ensuring that you receive the best care possible while addressing any concerns you may have. Remember, prioritizing your health is vital, and participating in regular screenings is a significant step in that direction.
