How to care for skin issues?
Hello Doctor: I am a 27-year-old male.
I previously visited a dermatologist for skin issues, and the doctor mentioned that I have naturally rough and sensitive skin, as well as more active sweat glands.
Recently, I have experienced the following two symptoms: 1.
I have developed numerous small dark red spots on my chest, which improve after showering and cooling down (but still persist).
I'm unsure if this is simply heat rash or an allergic reaction.
2.
Occasionally, I notice small lumps (soft and painful when pressed) under my armpits and behind my ears.
Over the past few days, I have intentionally avoided using soap while showering, only rinsing with water, and both conditions have improved.
I would like to ask the doctor if there are any recommended soaps or body washes for my skin type, or if rinsing with water is sufficient? Should I be moisturizing regularly, and how should I care for my skin? Thank you.

Managing sensitive skin, especially when dealing with rashes and irritations, requires a careful and tailored approach. Given your history of skin sensitivity and the symptoms you've described, here are some skincare tips and recommendations to help you manage your condition effectively.
Understanding Your Symptoms
1. Chest Red Dots: The dark red spots on your chest that improve with cooling could be indicative of heat rash (miliaria) or an allergic reaction. Heat rash occurs when sweat ducts become blocked, leading to small, red bumps. If these spots persist or worsen, it may be worth consulting a dermatologist for a definitive diagnosis.
2. Soft, Painful Bumps: The small, painful lumps in your armpits and behind your ears could be lymph nodes reacting to irritation or infection, or they could be cysts or folliculitis (inflammation of hair follicles). Avoiding soap and using just water seems to have provided some relief, which suggests that your skin may be reacting to certain ingredients in soaps or body washes.
Skincare Recommendations
1. Gentle Cleansing: Since your skin is sensitive, opt for a mild, fragrance-free cleanser. Look for products labeled as "hypoallergenic" or "for sensitive skin." Avoid harsh soaps that can strip your skin of its natural oils. A gentle, non-foaming cleanser can help maintain your skin's barrier while effectively removing dirt and sweat.
2. Moisturizing: Hydration is crucial for sensitive skin. After cleansing, apply a fragrance-free moisturizer that contains soothing ingredients like ceramides, hyaluronic acid, or glycerin. These ingredients help to restore the skin barrier and retain moisture. Consider using a thicker cream or ointment if your skin feels particularly dry or irritated.
3. Avoid Irritants: Stay away from products that contain alcohol, fragrances, or strong exfoliants, as these can exacerbate sensitivity. When trying new products, patch test them on a small area of skin before applying them more broadly.
4. Sun Protection: Protecting your skin from UV rays is essential, especially if you have sensitive skin. Use a broad-spectrum sunscreen with SPF 30 or higher daily, even on cloudy days. Look for physical (mineral) sunscreens containing zinc oxide or titanium dioxide, as they are less likely to irritate sensitive skin.
5. Clothing Choices: Wear loose-fitting, breathable fabrics to minimize irritation from sweat and friction. Cotton is a good choice, as it allows your skin to breathe.
6. Hydration and Diet: Staying hydrated and maintaining a balanced diet rich in antioxidants can support skin health. Foods high in omega-3 fatty acids, such as fish and flaxseeds, can also help reduce inflammation.
7. Consult a Dermatologist: Since you have a history of skin issues, it’s advisable to consult a dermatologist for a personalized skincare regimen. They can provide specific recommendations based on your skin type and any underlying conditions.
Conclusion
Managing sensitive skin requires a gentle approach and a focus on hydration and protection. By choosing the right products and being mindful of your skin's reactions, you can help minimize irritation and maintain healthy skin. If your symptoms persist or worsen, seeking professional medical advice is crucial to ensure appropriate treatment and care.
