Left lower abdominal pain?

Hello doctor, I had fried and very spicy food for dinner today.
After that, when I lie down or press on my lower left abdomen, it hurts, but there is no problem when I stand.
When I press on it, I can feel a throbbing sensation in my lower left abdomen.
After resting for a while, it gradually got better.
I would like to know what the issue might be.

Yun, 10~19 year old female. Ask Date: 2019/08/09

Dr. Ke Fangxu reply Surgery

It should just be indigestion combined with the irritation from capsaicin.
I recommend that you stick to a light diet for the next couple of days, and as long as you don't experience any more abdominal pain, you should be fine.

Left lower abdominal pain can arise from various causes, and understanding the underlying reasons is crucial for effective management. Based on your description, it appears that the pain is associated with specific activities, such as lying down or applying pressure to the area, and it improves when you are standing. This pattern can provide valuable insights into the potential causes of your discomfort.

One common cause of left lower abdominal pain is gastrointestinal issues. Given that you mentioned consuming fried and spicy food for dinner, it is possible that your symptoms are related to digestive disturbances. Fried foods can be difficult to digest, and spicy foods can irritate the gastrointestinal tract, leading to symptoms such as cramping or discomfort. The sensation of your left lower abdomen "jumping" could be related to increased intestinal activity or spasms in the colon, particularly if you have a condition like irritable bowel syndrome (IBS) or even mild gastritis.

Another consideration is the possibility of diverticulitis, especially if you have a history of diverticular disease. This condition occurs when small pouches (diverticula) in the colon become inflamed or infected, leading to localized pain, often in the left lower quadrant. However, diverticulitis typically presents with more persistent pain, fever, and changes in bowel habits, so your symptoms may not align perfectly with this diagnosis.

Musculoskeletal issues could also be a factor. If you experience pain when pressing on the area, it could indicate a muscle strain or irritation of the abdominal wall muscles. Activities that involve bending or twisting can exacerbate such pain, and it may feel better when standing or in certain positions.

In women, gynecological conditions must also be considered. Ovarian cysts, endometriosis, or other reproductive system issues can manifest as left lower abdominal pain. If your symptoms coincide with your menstrual cycle or if you have other gynecological symptoms, it would be prudent to consult a gynecologist for further evaluation.

Given that your pain improved after resting, it suggests that it may not be a severe underlying condition. However, if the pain persists, worsens, or is accompanied by other symptoms such as fever, vomiting, or changes in bowel habits, it is essential to seek medical attention. A healthcare provider may recommend imaging studies, such as an ultrasound or CT scan, to rule out any serious conditions.

In summary, left lower abdominal pain can stem from various causes, including gastrointestinal disturbances, musculoskeletal issues, or gynecological conditions. Your recent dietary choices may have contributed to your symptoms, but it is essential to monitor your condition and consult a healthcare professional if the pain continues or worsens. Keeping a symptom diary, noting when the pain occurs, its intensity, and any associated symptoms can also help your healthcare provider make a more accurate diagnosis.
