Yaz contraceptive pill
Hello Dr.
Huang, I would like to ask you some questions regarding the Yaz contraceptive pill.
My menstrual cycle is consistently around 27/28 days, and I have only engaged in foreplay with my boyfriend, without any sexual intercourse.
I started taking the 24+4 Yaz pill on the first day of my last menstrual period, July 24, and I take it at the same time every day.
I expect to finish the pink pills on August 16 and the last four white pills by August 20.
I have a few questions:
1) During the last four white pills (August 17-20), will I still have contraceptive effectiveness?
2) When is the correct time to start the second pack of Yaz? Is it on the first day of my next period or the fifth day? If I start on the fifth day, does that mean there will be no contraceptive effect during the first four days without taking the pill? (Since we are in a long-distance relationship, it might happen that we can only meet during those few days...)
3) If my period does not arrive by August 21 after finishing the white pills, should I continue to wait for my period, or should I start the second pack immediately? If there is a gap without taking the pill, will it lose its contraceptive effectiveness? What is the safest way to take it?
4) In high school, I experienced a lot of stress and had a yeast infection once.
Since then, I have been taking cranberry supplements, and recently I started adding probiotics (because since the 16th day of taking Yaz until now on the 20th day, I occasionally feel itching in the vulvar area and am very concerned about getting infected again).
I understand that taking contraceptive pills may increase the risk of vaginal/urethral infections.
Could you please advise me on how to maintain and prevent this?
Thank you for taking the time to respond.

Dr. Huang Yiwen reply Obstetrics and Gynecology
Hello, Anonymous Sister: The Yaz birth control pill is designed with a regimen of 24 active pills and 4 inactive pills to indicate that after finishing the total of 28 pills, you should continue to the next pack regardless of which day of your menstrual cycle it is, without any break in between.
The last four white tablets do not contain any contraceptive ingredients; they simply serve to ensure that there is no gap in your medication schedule, helping you remember to start the next pack on time.
Taking birth control pills does not increase the risk of urinary tract or vaginal infections.
It is important to maintain a regular lifestyle, avoid staying up late, engage in regular exercise, wear breathable clothing, and drink an adequate amount of water, as these practices can significantly reduce the risk of urinary tract or vaginal infections.

When it comes to understanding Yaz birth control, it's essential to clarify how it works, its effectiveness, and the best practices for its use. Yaz is a combination oral contraceptive pill that contains ethinyl estradiol and drospirenone. It is designed to prevent ovulation, thicken cervical mucus, and thin the uterine lining, making it an effective method of birth control when taken correctly.
1. Effectiveness During the White Pill Phase (8/17-8/20): The last four white pills in the Yaz pack are placebo pills, meaning they do not contain active hormones. However, they are included to help maintain a routine of taking a pill daily. During this time, you still have contraceptive protection as long as you have taken the previous active pills correctly. Therefore, if you have been taking the pink pills consistently and on time, you will remain protected from pregnancy even during the placebo phase.
2. Starting the Second Pack: The correct time to start the second pack of Yaz is immediately after finishing the first pack, including the placebo pills. You do not need to wait for your period to start the next pack. If you begin the second pack on the day after finishing the white pills (8/21), you will maintain continuous contraceptive coverage. If you were to wait until your next period (which may not arrive immediately), you would have a gap in protection, which could increase the risk of unintended pregnancy.
3. What to Do If Your Period Doesn't Start After the White Pills: If you finish the white pills and do not get your period, you should still start the next pack of Yaz. It is common for periods to become lighter or even skip while on the pill, especially after several months of consistent use. However, if you miss your period for two consecutive cycles, it is advisable to take a pregnancy test to rule out pregnancy, as this could indicate a need for further evaluation.
4. Preventing Infections While on Yaz: Regarding your concerns about vaginal or urinary tract infections, it is important to note that while some studies suggest hormonal contraceptives may alter the vaginal flora, they do not significantly increase the risk of infections. To maintain good vaginal health, consider the following tips:
- Hygiene: Maintain proper hygiene by washing the genital area with mild soap and water. Avoid douching, as it can disrupt the natural balance of bacteria.
- Clothing: Wear breathable cotton underwear and avoid tight-fitting clothing to reduce moisture buildup.
- Hydration: Drink plenty of water to help flush out your urinary tract.
- Probiotics: Continuing with probiotics can be beneficial for maintaining a healthy balance of bacteria in your body.
- Cranberry Supplements: These can help prevent urinary tract infections, but they should not replace medical treatment if an infection occurs.
In summary, Yaz is a highly effective contraceptive when taken as directed. You can continue to take it even if your period does not arrive after the placebo pills. Always consult your healthcare provider if you have any concerns about side effects or if you suspect you might be pregnant. Regular check-ups and open communication with your healthcare provider are key to ensuring your reproductive health while using birth control.
