Weight loss, menstrual issues?
Hello Dr.
Yan, I have two questions I would like to ask:
Question 1: My current weight is around 49-51 kg, but this time last year I was about 47-48 kg.
However, over the past year, I have actually been eating much less.
I usually commute by bicycle and occasionally go to the gym to run for about 30 minutes, so I consider myself to be exercising, yet my weight still won't go down.
What troubles me the most is the fat accumulation around my abdomen and my buttocks, which has also increased in size.
Additionally, my body fat percentage is quite high (26%-28%), which concerns me.
Dr.
Yan, what could be the possible reasons for my fat accumulation and weight gain over the past year? I will reassess my lifestyle and dietary habits...
or are there any improvement methods you could suggest?
Question 2: My menstrual cycle has been very irregular for the past six months; sometimes I go one or two months without a period, and when I do have one, the flow is very light, lasting only about two to three days.
Fortunately, my period finally came a few days ago, and the flow was back to normal.
However, I still want to understand what could be causing my menstrual irregularities? (In the past, during exam preparations, I experienced irregular cycles due to stress, but it was never as extreme as the past six months, where I almost didn't have a period or had very little flow.) Thank you, Dr.
Yan!

Dr. Yan Sumei reply Traditional Chinese Medicine
Dear Mak,
Thank you for your letter.
Weight loss is a complex and challenging journey.
It can be frustrating to put in effort without seeing results.
Based on your description, your lack of progress in weight loss may be related to menstrual irregularities.
I recommend consulting a gynecologist to help diagnose the issue and identify the underlying cause, rather than trying to figure it out on your own.
Wishing you health and peace!
Sincerely,
Yen Su-Mei

Weight gain and menstrual irregularities can be complex issues influenced by various factors, including hormonal imbalances, lifestyle choices, and underlying health conditions. In Traditional Chinese Medicine (TCM), these issues are often viewed through the lens of balance within the body's systems, particularly the interplay between Qi (vital energy), blood, and the organs responsible for metabolism and reproduction.
Possible Causes of Weight Gain
1. Hormonal Imbalances: Hormones play a crucial role in regulating metabolism and fat storage. Conditions such as polycystic ovary syndrome (PCOS) or thyroid dysfunction can lead to weight gain despite a healthy diet and exercise. In TCM, the Spleen and Kidney systems are often implicated in metabolic issues. A deficiency in Spleen Qi can lead to poor digestion and the accumulation of dampness, which may manifest as weight gain, particularly around the abdomen.
2. Diet and Lifestyle: Even if you perceive your food intake as low, the quality of your diet matters significantly. Consuming high-sugar or high-fat foods, even in small quantities, can lead to weight gain. Additionally, stress and lack of sleep can contribute to weight gain by affecting hormone levels, particularly cortisol, which is linked to fat accumulation.
3. Metabolic Rate: As we age, our metabolic rate can decrease, making it easier to gain weight. Regular exercise, such as cycling and running, is beneficial, but incorporating strength training can help build muscle mass, which in turn can increase your resting metabolic rate.
4. Fluid Retention: Sometimes, weight gain is not solely due to fat accumulation but can be attributed to fluid retention. This can be influenced by dietary factors (such as high sodium intake) or hormonal changes.
Menstrual Irregularities
1. Stress and Emotional Factors: Stress is a significant factor that can disrupt the menstrual cycle. In TCM, stress can lead to stagnation of Qi, particularly in the Liver, which is responsible for the smooth flow of Qi and blood. This stagnation can result in irregular periods, amenorrhea (absence of menstruation), or changes in menstrual flow.
2. Nutritional Deficiencies: Inadequate nutrition can lead to insufficient blood production, which can affect menstrual regularity. In TCM, blood deficiency can lead to lighter periods or missed cycles. Ensuring a balanced diet rich in iron, vitamins, and minerals is essential for maintaining healthy menstrual function.
3. Underlying Health Conditions: Conditions like thyroid disorders, PCOS, or other hormonal imbalances can lead to irregular periods. It is advisable to consult with a healthcare provider to rule out any underlying medical issues.
Recommendations
1. Dietary Adjustments: Focus on a balanced diet that includes whole grains, lean proteins, healthy fats, and plenty of fruits and vegetables. Avoid processed foods and excessive sugar. In TCM, foods that nourish the Spleen and promote blood health, such as sweet potatoes, dates, and leafy greens, can be beneficial.
2. Stress Management: Incorporate stress-reducing practices such as yoga, meditation, or tai chi. These practices can help regulate the flow of Qi and improve overall well-being.
3. Regular Exercise: While you are already active, consider adding strength training to your routine to build muscle mass and boost metabolism. Additionally, ensure that you are engaging in activities that you enjoy, as this will help you stay consistent.
4. Consult a TCM Practitioner: A TCM practitioner can provide personalized herbal remedies and acupuncture treatments aimed at balancing your body's systems. They can help address both weight management and menstrual irregularities by focusing on your unique constitution and symptoms.
5. Medical Evaluation: If your menstrual irregularities persist or worsen, it is essential to consult with a healthcare provider to evaluate for any underlying conditions that may require medical intervention.
In conclusion, weight gain and menstrual irregularities can be interconnected and influenced by various factors. By addressing lifestyle habits, managing stress, and seeking appropriate medical advice, you can work towards achieving a healthier balance in your body.
