Urinary Symptoms: Is It Kidney Stones or Anxiety? - Urology

I'm sorry, but I cannot assist with that?

Hello Doctor: After experiencing hematuria that day, I took Buscopan and the hematuria stopped.
However, today during urination, I suddenly felt tightness in the perineum, my penis became erect, the urine flow decreased, and I experienced some pain after urination.
When I contract my anal sphincter after urination, I feel pain in the perineum.
Could this be a sign of a stone being lodged somewhere, or is it just pain in the perineum due to anxiety?

Tt, 20~29 year old female. Ask Date: 2019/09/30

Hello: If similar symptoms occur each time, it may be at the distal end of the ureter or the bladder opening.

Understanding urinary symptoms can be quite complex, especially when differentiating between conditions like kidney stones and anxiety-related issues. Based on your description, it seems you are experiencing a combination of urinary symptoms, including hematuria (blood in urine), perineal tightness, changes in urinary flow, and post-void pain. Let's break down these symptoms and their potential causes.

1. Kidney Stones: Kidney stones can cause a variety of symptoms, including severe pain, hematuria, and changes in urinary habits. When a stone moves through the urinary tract, it can cause pain that radiates from the back to the lower abdomen and groin. If a stone is lodged in the urethra or bladder, it can lead to urinary retention, difficulty urinating, and even a feeling of pressure or tightness in the perineal area. The fact that you experienced blood in your urine could indicate that a stone has caused irritation or injury to the urinary tract lining.

2. Anxiety and Stress: Anxiety can manifest in physical symptoms, including muscle tension, which may lead to a sensation of tightness in the pelvic area. Stress can also exacerbate urinary symptoms, leading to increased frequency, urgency, and discomfort during urination. The tightness and discomfort you feel in the perineal region could be related to muscle tension rather than a physical obstruction like a kidney stone.

3. Differentiating Symptoms: The key to understanding whether your symptoms are due to kidney stones or anxiety lies in the specifics of your experience:
- Pain Characteristics: Kidney stone pain is typically sharp and severe, often described as colicky, and may come in waves. If your pain is more of a dull ache or tightness, it could be related to muscle tension.

- Urinary Flow Changes: A decrease in urinary flow can occur with kidney stones, especially if they are obstructing the urethra. If you notice a significant change in your ability to urinate, it warrants further investigation.

- Post-void Pain: Pain after urination can be associated with urinary tract infections, bladder irritation, or stones. If this symptom persists, it is essential to consult a healthcare provider.

4. Next Steps: Given your symptoms, it is advisable to seek medical attention. A healthcare provider may recommend imaging studies, such as an ultrasound or CT scan, to check for the presence of kidney stones. Additionally, a urinalysis can help identify any signs of infection or other abnormalities.

5. Managing Anxiety: If anxiety is contributing to your symptoms, consider discussing this with your healthcare provider. They may suggest strategies for managing anxiety, such as cognitive-behavioral therapy, relaxation techniques, or medication if necessary.

In summary, while your symptoms could indicate the presence of kidney stones, they may also be influenced by anxiety and muscle tension. A thorough evaluation by a healthcare professional is essential to determine the underlying cause and appropriate treatment. Remember, it is crucial to address both physical and mental health aspects to achieve overall well-being.
