Changes in Ejaculation: From Forceful to Dribbling - What’s Happening? - Urology

Previously, ejaculation was characterized by a jet-like spray, but now it is more of a flowing discharge?

Hello Doctor, I have recently noticed that during masturbation, the ejaculation of semen often flows out or only shoots a little before tapering off.
In the past, even with daily masturbation, it would spray out.
However, I only recently became aware of this issue and I'm not sure how long it has been happening.
In the past few months, I have undergone a circumcision and frenuloplasty, and I also started dating a girlfriend.
When I'm with her, I often have prolonged erections.
I'm wondering if this could be due to a decrease in excitement for masturbation, or if it might be related to the circumcision.
Thank you, Doctor.

Hello: It is more likely due to excessive ejaculation or masturbation; circumcision should not be related.
You might try doing some exercises to strengthen your pelvic floor muscles.
Since you are very young, you should recover well.
Wishing you good health.

Changes in ejaculation, particularly a shift from forceful ejaculation to a dribbling or less forceful release, can be concerning for many individuals. This change can stem from various factors, including physiological changes, psychological influences, and recent medical procedures, such as circumcision or frenuloplasty.

Firstly, it’s essential to understand that ejaculation involves a complex interplay of muscular contractions, hormonal signals, and neurological responses. The seminal vesicles and prostate gland contribute to the volume and force of the ejaculate. When you notice a change in the force of your ejaculation, it could be due to several reasons:
1. Physiological Changes: As men age, changes in hormone levels, particularly testosterone, can affect sexual function, including the force of ejaculation. Additionally, conditions such as pelvic floor dysfunction or issues with the prostate can lead to changes in ejaculatory force.

2. Psychological Factors: Stress, anxiety, and changes in sexual arousal can significantly impact sexual performance. If you have recently entered a new relationship, the excitement and nervousness associated with it could alter your sexual responses, including ejaculation. The excitement of being with a partner may lead to different arousal patterns compared to solo activities like masturbation.

3. Impact of Medical Procedures: You mentioned undergoing circumcision and frenuloplasty. These procedures can lead to changes in sensitivity and sexual response. After circumcision, some men report changes in sensation, which can affect arousal and ejaculation. The frenulum, which is a sensitive band of tissue on the underside of the penis, plays a significant role in sexual pleasure. Alterations to this area can lead to changes in how you experience ejaculation.

4. Frequency of Sexual Activity: If you are engaging in sexual activity with your girlfriend frequently, this could also influence your experience during masturbation. The body may adapt to different types of stimulation, and this could lead to a perceived decrease in force during ejaculation when you return to masturbation.

5. Health Considerations: While less common, certain medical conditions can affect ejaculation. For example, infections, hormonal imbalances, or neurological issues can lead to changes in ejaculatory function. If you notice persistent changes or if they are accompanied by other symptoms (such as pain, swelling, or changes in libido), it would be wise to consult a healthcare professional.

In conclusion, while changes in ejaculation can be concerning, they are often influenced by a combination of physiological, psychological, and situational factors. If the change in your ejaculation is accompanied by other symptoms or if it persists, it is advisable to seek medical advice. A healthcare provider can perform a thorough evaluation to rule out any underlying conditions and provide guidance tailored to your specific situation. Remember, open communication with your partner and healthcare provider can also help alleviate concerns and improve your sexual health.

Dr. Xu Weikai reply Urology
Xiao Yi: The force of ejaculation involves many factors, including the strength of the erection, the volume of semen, and the intensity of stimulation. Additionally, the method of ejaculation does not have a significant impact on future chances of conception.
