Black lines on toenails?
Hello, doctor.
I noticed a faint black line, about 0.3 centimeters long and vertical, appearing near the tip of my right big toe over the past two days.
I have been running these last couple of days and I'm worried it might be melanoma.
Is it possible that dirt got in there?

The appearance of black lines on toenails can be concerning, especially when they appear suddenly or change in appearance. In your case, the faint black line on your right big toe, which is approximately 0.3 cm long and vertical, could be attributed to several factors. While it is understandable to worry about conditions such as melanoma (a type of skin cancer), it is essential to consider other possible explanations as well.
Firstly, one common cause of black lines or streaks under the nails is a condition known as "splinter hemorrhages." These are tiny blood clots that can occur due to trauma, such as running or stubbing your toe. If you have recently engaged in activities that put pressure on your toenails, such as running, it is possible that the black line is a result of minor trauma rather than a serious condition. Splinter hemorrhages typically appear as thin, vertical lines and can be harmless, especially if they are not accompanied by other symptoms like pain or swelling.
Another possibility is the presence of a fungal infection, which can sometimes cause discoloration of the nails. Fungal infections can lead to changes in nail color, texture, and thickness. However, these infections usually present with additional symptoms, such as itching or a foul odor, which you have not mentioned.
Regarding your concern about dirt or debris getting trapped under the nail, while this is theoretically possible, it is less common for it to cause a distinct black line. More often, dirt may cause a general discoloration rather than a defined line.
Melanoma, particularly subungual melanoma (which occurs under the nail), is a rare but serious condition that can present as a dark streak or line. It is crucial to monitor the line for changes in color, width, or if it begins to spread. If the line persists, changes, or if you notice any other concerning symptoms (such as pain, swelling, or changes in the surrounding skin), it is advisable to seek medical attention promptly.
In summary, while the appearance of a black line on your toenail can be alarming, it is essential to consider the context of recent activities and any accompanying symptoms. If the line does not resolve or if you have any concerns about its appearance, it is best to consult a healthcare professional, preferably a dermatologist, who can perform a thorough examination and determine if any further investigation or treatment is necessary. Regular monitoring and early intervention are key to addressing any potential issues effectively.
