Sudden blurred vision in the left eye upon waking up?

Hello doctor, today when I woke up, my left eye suddenly had blurred vision (like looking through a veil) for about 3 to 5 minutes.
Could this be related to dry eye syndrome? I have been suffering from dry eye syndrome for ten years, but this is the first time I have experienced this situation.
I also have high myopia and am concerned that there might be some pathological changes.
However, I just had an OCT examination last month, and the doctor said there were no significant issues at that time.
Thank you.

Dr. Cai Wenyuan reply Ophthalmology

Hello: If you have pressed on your eyeball, observe the situation.
If the symptoms persist, seek medical attention.

Sudden blurry vision, especially upon waking, can be concerning and may have various underlying causes. In your case, where you have a history of dry eye syndrome and high myopia, it is essential to consider both your existing conditions and the sudden change in vision.

Firstly, dry eye syndrome can lead to temporary visual disturbances. When you wake up, your eyes may not have been adequately lubricated during sleep, especially if you sleep with your eyes partially open or in a dry environment. This lack of moisture can cause the tear film to become unstable, leading to blurry vision that may resolve after blinking or after a few minutes as tears spread across the surface of the eye. However, since you mentioned that this is the first time you have experienced such a symptom in ten years, it may warrant further investigation.

Another possibility is that your blurry vision could be related to your high myopia. Individuals with high myopia are at an increased risk for various ocular complications, including retinal detachment, macular degeneration, and other structural changes in the eye. Although your recent OCT (Optical Coherence Tomography) scan indicated no significant issues, it is still important to remain vigilant about any new symptoms, especially sudden changes in vision.

Additionally, the phenomenon you described could also be linked to a transient condition known as "amaurosis fugax," which refers to a temporary loss of vision due to a lack of blood flow to the retina. This condition typically lasts for a few seconds to minutes and can be a warning sign of more serious cardiovascular issues. However, given your description of the vision being blurry rather than completely lost, this may be less likely.

Other potential causes for sudden blurry vision upon waking could include:
1. Corneal Edema: This occurs when fluid accumulates in the cornea, often due to overnight pressure on the eye or conditions like Fuchs' dystrophy. This can lead to temporary blurriness that may improve with blinking or after some time.

2. Retinal Issues: Although your OCT was normal, conditions such as retinal tears or detachments can sometimes present with sudden changes in vision. If you experience any flashes of light or floaters, it is crucial to seek immediate medical attention.

3. Neurological Factors: Sometimes, visual disturbances can be linked to neurological issues, including migraines or transient ischemic attacks (TIAs). If you have a history of migraines, this could be a contributing factor.

4. Environmental Factors: Changes in lighting or exposure to bright light upon waking can also cause temporary visual disturbances.

Given your concerns and the sudden nature of your symptoms, it is advisable to consult with an ophthalmologist or a neuro-ophthalmologist. They can perform a comprehensive examination, including visual field tests and possibly imaging studies, to rule out any serious underlying conditions. Keeping a record of when these episodes occur, their duration, and any accompanying symptoms can also be helpful for your healthcare provider in diagnosing the issue.

In summary, while your dry eye syndrome may contribute to temporary blurry vision, the sudden onset of this symptom, especially in the context of high myopia, warrants further evaluation to ensure there are no serious underlying conditions. Regular follow-ups with your eye care provider are essential to monitor your eye health, especially given your history of high myopia and dry eyes.
