Menstrual issues
Hello Doctor: My menstrual period started on September 16 and lasted for 7 days with a normal flow.
However, this month on October 18, I experienced a slight brown discharge for 4 consecutive days (during this time, I had significant lower back pain, abdominal discomfort, and excessive sleepiness).
I have polycystic ovary syndrome (PCOS) and have been taking traditional Chinese medicine (herbal decoction) for 4 months, during which I have had regular periods for 4 months.
Before starting the herbal treatment, I had irregular menstrual cycles.
I took pregnancy tests on October 14, 15, and 17, but all showed only one line.
On October 17, a doctor at Chung Shan Medical University did not perform an ultrasound to check my uterine condition.
I would like to ask the doctor: given my current situation, is this considered a menstrual period or is there another issue? Is it possible that I am in the early stages of pregnancy and the HCG levels are too low to detect?

Dr. Huang Yiwen reply Obstetrics and Gynecology
Hello, Miss Xiao Jia: Your situation is most likely due to menstrual irregularities, leading to a decreased menstrual flow.
This menstrual period is likely an anovulatory cycle.
As for your concern about pregnancy, since the urine test showed only one line, the likelihood is extremely low.
I suggest you wait patiently for another week.
If your period still does not arrive, you can take another urine test or a blood test to find out the answer.

More Info
Irregular menstrual bleeding can be a source of concern for many women, and understanding the potential causes and implications is crucial for managing one's health. Based on your description, it seems that you experienced a normal menstrual cycle in September, but in October, you encountered some unusual symptoms, including light brown discharge and abdominal discomfort. Let's break down the possible causes and considerations regarding your situation.
Possible Causes of Irregular Menstrual Bleeding
1. Hormonal Imbalances: Given your history of polycystic ovary syndrome (PCOS), hormonal fluctuations are a common issue. PCOS can lead to irregular menstrual cycles, and even when you have been experiencing regular periods due to herbal treatment, it is possible for hormonal imbalances to still cause occasional irregularities.
2. Implantation Bleeding: If there is a possibility of pregnancy, light spotting can occur when a fertilized egg implants itself into the uterine lining. This typically happens around the time your next period would be due, which could explain the brown discharge you observed.
3. Stress and Lifestyle Factors: Stress, changes in weight, and lifestyle factors can significantly impact menstrual cycles. If you have experienced any recent changes in your life, such as increased stress or changes in diet or exercise, these could contribute to irregular bleeding.
4. Infections or Other Medical Conditions: Infections, such as pelvic inflammatory disease (PID), or other conditions affecting the reproductive system can also lead to irregular bleeding. If you have any additional symptoms, such as fever or unusual discharge, it would be wise to consult a healthcare provider.
5. Pregnancy: As you mentioned, you have taken pregnancy tests that returned negative results. However, it is important to note that early pregnancy tests can sometimes yield false negatives, especially if taken too early. If you suspect pregnancy, consider waiting a few days and testing again, or consult with your healthcare provider for a blood test, which is more sensitive.
Concerns and Recommendations
- Monitoring Symptoms: Keep track of your menstrual cycle, including the duration and nature of bleeding, as well as any accompanying symptoms like pain or fatigue. This information can be valuable for your healthcare provider.
- Consulting a Healthcare Provider: Since you have a history of PCOS and are experiencing changes in your menstrual cycle, it is advisable to consult a healthcare provider. They may recommend further evaluation, such as an ultrasound or blood tests, to assess hormone levels and the health of your reproductive organs.
- Lifestyle Adjustments: Maintaining a healthy lifestyle, including a balanced diet, regular exercise, and stress management techniques, can help regulate menstrual cycles. If you are experiencing significant stress or lifestyle changes, consider seeking support or counseling.
- Follow-Up on Herbal Treatments: While herbal treatments can be beneficial for some, it is essential to discuss any ongoing treatments with your healthcare provider to ensure they are appropriate for your current health status and do not interfere with other treatments.
In summary, while your symptoms could be related to hormonal imbalances, potential pregnancy, or other factors, it is crucial to seek professional medical advice to determine the underlying cause and appropriate management. Regular monitoring and open communication with your healthcare provider will help ensure your reproductive health is well-managed.
