Penile issues
Hello! My foreskin was red and swollen for two days, and although the swelling has subsided, there are now two or three straight black marks on the skin of my penis, covering about 1/4 of the circumference at the front (not encircling the entire shaft).
Additionally, there are white flakes on half of the skin of my scrotum.
Urination is normal, with no discharge or bleeding.
There is no pain or itching.
Could this be a fungal infection, or is there another possibility?
Tieganren, 50~59 year old female. Ask Date: 2019/10/27
Dr. Wu Yuxin reply Dermatology
Without actually seeing the lesion, it is impossible to determine the issue.
If it is causing significant distress, please seek medical attention as soon as possible.
A proper examination is necessary to understand the problem and to determine the appropriate treatment.

When it comes to penile skin issues, there are several potential causes and concerns that could lead to symptoms like redness, swelling, and discoloration. Based on your description, it seems you experienced some inflammation followed by the appearance of dark streaks on the skin and white flakes on the scrotum. Let's explore these symptoms in detail.
Firstly, the redness and swelling of the foreskin could be indicative of a condition known as balanitis, which is inflammation of the glans (the head of the penis) and can occur due to various reasons, including infections (bacterial or fungal), irritants (such as soaps or lotions), or even allergic reactions. Given that you mentioned the absence of pain, discharge, or bleeding, it is less likely to be a severe infection, but it is still important to consider the possibility of a fungal infection, particularly if you have a history of such issues.
The appearance of dark streaks on the penile skin could be due to several factors. One possibility is post-inflammatory hyperpigmentation, which can occur after an inflammatory process has resolved. This is a common response of the skin, where it darkens in areas that were previously inflamed. Another possibility could be a benign condition such as pearly penile papules or other harmless skin variations, but these typically do not present as linear streaks.
The white flakes on the scrotum may suggest a condition like seborrheic dermatitis, which can cause flaking and scaling of the skin. This condition is often exacerbated by dryness or irritation and can be treated with appropriate topical treatments.
In terms of your concern about a fungal infection, candidiasis (a type of yeast infection) is a common cause of penile skin issues, especially if there are risk factors such as moisture, heat, or a compromised immune system. Symptoms of a fungal infection can include redness, itching, and a white discharge, but since you mentioned no discharge or pain, it may not be the primary concern.
Given your symptoms, here are some recommendations:
1. Maintain Hygiene: Ensure that you keep the area clean and dry. Use mild, unscented soap and avoid harsh chemicals or irritants that could exacerbate the condition.
2. Moisturize: If the skin appears dry or flaky, consider using a gentle, fragrance-free moisturizer to help alleviate dryness.
3. Avoid Irritants: Steer clear of any products that may irritate the skin, including certain soaps, lotions, or laundry detergents.
4. Monitor Symptoms: Keep an eye on the symptoms. If the discoloration or flaking persists or worsens, it would be wise to consult a healthcare professional for a thorough examination.
5. Seek Medical Advice: Since you are experiencing unusual symptoms, it is advisable to see a dermatologist or urologist. They can provide a proper diagnosis and treatment plan tailored to your specific condition.
In conclusion, while your symptoms may not immediately suggest a serious condition, they warrant attention to rule out any underlying issues. A healthcare professional can help determine the exact cause of your symptoms and recommend appropriate treatment options. Remember, early intervention can often lead to better outcomes, so don’t hesitate to seek medical advice if you have concerns about your skin health.
