Penile itching

Hello, recently I've been experiencing frequent itching on my penis, particularly concentrated around the scar from my previous circumcision.
It doesn't itch when I don't touch it, but it does when I do.
There's no discharge or open wounds, and it doesn't appear red or swollen; it looks completely normal.
I haven't changed my body wash, and my lifestyle habits are normal.
It's been over four years since my circumcision, and this is the first time this has happened.
What could be the possible cause? I'm currently abroad and plan to seek medical attention when I return to Taiwan, but that will take some time.
Is there any topical ointment I can buy in the meantime? Thank you.

Josh, 20~29 year old female. Ask Date: 2019/11/04

Hello: There is no redness (redness may indicate inflammation, which would require anti-inflammatory ointment).
Currently, just pay attention to your lifestyle: wear loose-fitting and breathable underwear, keep the area dry, and avoid hot water during showers.
There is no need to use ointment.
You may consider temporarily not using body wash; rinsing with clean water and drying off is sufficient.
Wishing you good health.

More Info

Itchy sensations in the genital area, particularly in the region of a circumcision scar, can be quite uncomfortable and concerning. There are several potential causes for this itching, and understanding them can help you find temporary relief while you await a visit to a healthcare professional.

Possible Causes of Itchy Penis
1. Irritation or Allergic Reaction: This is one of the most common causes of itching. It could be due to an allergic reaction to soaps, lotions, or even laundry detergents. Even if you haven't changed your products recently, it’s possible that your skin has developed a sensitivity over time.

2. Dry Skin: The skin in the genital area can become dry, especially if exposed to harsh soaps or if you live in a dry climate. This dryness can lead to itching.

3. Infections: While you mentioned there is no discharge or visible redness, it’s still important to consider the possibility of a fungal infection (like jock itch) or a yeast infection, which can cause localized itching.

4. Scar Tissue: Since you mentioned that the itching is concentrated around the circumcision scar, it’s possible that the scar tissue is more sensitive or has developed some irritation.

5. Dermatitis: Conditions such as contact dermatitis can occur when the skin reacts to irritants or allergens. This can happen even with products that you have used for a long time without issue.

6. Sexually Transmitted Infections (STIs): Although you do not report any other symptoms, it’s important to consider STIs as a potential cause of genital itching.

Temporary Relief Options
While you are waiting to see a healthcare provider, there are some over-the-counter options and home remedies you can consider to alleviate the itching:
1. Topical Hydrocortisone Cream: This can help reduce inflammation and itching. It’s important to use it sparingly and not for extended periods, as prolonged use can thin the skin.

2. Moisturizers: Applying a gentle, fragrance-free moisturizer can help combat dryness. Look for products specifically designed for sensitive skin.

3. Antifungal Creams: If you suspect a fungal infection, over-the-counter antifungal creams (like clotrimazole or miconazole) can be applied to the affected area.

4. Avoid Irritants: Try to avoid any potential irritants, including scented soaps or lotions. Stick to mild, unscented products.

5. Cold Compress: Applying a cold compress to the area can provide temporary relief from itching.

6. Loose-fitting Clothing: Wearing loose-fitting underwear and clothing can help reduce irritation and allow the area to breathe.

When to Seek Medical Attention
Since you plan to see a doctor upon your return to Taiwan, it’s wise to keep that appointment. However, if you experience any of the following symptoms before then, you should seek medical attention sooner:
- Persistent or worsening itching
- Development of a rash, redness, or swelling
- Any discharge from the penis
- Pain or discomfort during urination or sexual activity
In conclusion, while there are several potential causes for your symptoms, many can be managed temporarily with over-the-counter treatments. It’s crucial to monitor your symptoms and seek professional medical advice for a proper diagnosis and treatment plan.
