Prepuce issues
Hello, doctor.
I am 22 years old and have a few questions regarding my condition.
My penis size is normal both when flaccid and erect.
1.
When erect or flaccid, my foreskin does not retract automatically, but I can retract it manually.
I can also retract it while showering and during masturbation without it retracting back.
Does this mean I do not need a circumcision?
2.
When I retract the foreskin and put on a condom, it gradually becomes soft.
I have only been able to successfully penetrate my girlfriend a few times, but most of the time it goes soft.
I’m not sure why this happens, as my erection and function are normal during masturbation.
Could the condom be affecting this?
3.
Does my condition qualify as pseudophimosis? Would you recommend circumcision? I have previously seen a urologist who pulled back my foreskin and said it was normal and that I did not need a circumcision.
However, he did not ask me to retract it while erect.
I can manually pull the foreskin back to the coronal sulcus when erect without pain, but I am unsure how to overcome the issue with condoms.
4.
Is circumcision necessary for my foreskin condition, or can it be managed without surgery? Thank you, doctor, for your answers, and I appreciate your help.

Hello: 1) No need for circumcision.
2) It is related to erectile function and should not be an issue with condoms.
Sometimes, DIY practices can lead to certain habits, such as insufficient engorgement when stimulation is stopped.
If possible, try to minimize DIY, engage in moderate exercise, and maintain vascular engorgement capacity, which may help improve the situation.
3) Generally, those who are uncircumcised experience similar issues.
As long as the foreskin can retract, there is no constriction during erection, and it does not get stuck when retracted, it does not fall under the category for surgical recommendation.
4) Surgery is not necessary unless hygiene is difficult to maintain.
Wishing you good health.

When it comes to foreskin issues and the question of whether circumcision is necessary, there are several factors to consider. Based on your description, it seems that you have some concerns about your ability to retract the foreskin and the impact it has on sexual activity. Let’s address your questions one by one.
1. Foreskin Retraction: You mentioned that your foreskin does not retract automatically when flaccid or erect, but you can manually retract it without pain. This situation is often referred to as "physiological phimosis," which is common in younger males. Many individuals do not require circumcision if they can manually retract the foreskin without discomfort. The fact that you can retract it manually and that it does not cause pain is a positive sign. If you are able to maintain hygiene and there are no recurrent infections or discomfort, circumcision may not be necessary.
2. Erection and Condom Use: The issue of losing an erection when using a condom can be attributed to several factors. It may be related to anxiety about performance, the sensation of the condom, or even the fit of the condom itself. Some men find that certain types of condoms can feel too tight or uncomfortable, which can lead to loss of erection. It’s worth experimenting with different brands or types of condoms, including those designed to enhance sensitivity or those that are thinner. Additionally, practicing relaxation techniques and communicating with your partner can help alleviate performance anxiety.
3. Pseudo-phimosis: Based on your description, it sounds like you might be experiencing a form of pseudo-phimosis, where the foreskin can be retracted manually but does not do so naturally. This is not uncommon and does not necessarily require circumcision. If you can retract the foreskin without pain and maintain hygiene, it is generally considered normal. However, if you experience any pain, recurrent infections, or other complications, it would be advisable to consult a urologist for further evaluation.
4. Need for Circumcision: The decision to circumcise is highly personal and can depend on various factors, including medical necessity, cultural beliefs, and personal preference. Since your urologist has assessed your condition and deemed it normal, it suggests that circumcision may not be necessary at this time. If you have concerns about hygiene or recurrent infections, or if you find that the inability to retract the foreskin is causing significant issues in your sexual life, it may be worth revisiting the discussion with your healthcare provider.
In summary, based on your description, it seems that circumcision may not be necessary for you at this point. However, if you continue to experience difficulties with condom use or if you have concerns about your sexual health, it would be beneficial to have an open discussion with a urologist. They can provide tailored advice and explore options that may help improve your situation without the need for surgery. Remember, maintaining good hygiene and being comfortable with your body are key aspects of sexual health.
