Is it early contraction?
Hello doctor, I have a medical history of mitral valve prolapse and several premature contractions per day.
Today, it got cold, and while I was lying in bed, I first felt a skipped beat, then after coughing, I felt my heart pounding heavily three times, similar to a drumming sensation and also like a vibration.
I would like to ask if this is a premature contraction? Because previously, I only experienced the sensation of skipped beats, and I didn't have this heavy pounding feeling after a skipped beat today.
I was wondering if this could be atrial fibrillation and if it poses any danger?

Dr. He Dongjin reply Cardiology
Hello, it sounds more like premature contractions rather than atrial fibrillation.
The symptoms of feeling cold may be more pronounced, but there is no danger, so please don't worry.

Understanding early heart contractions, also known as premature contractions or premature beats, is crucial for assessing cardiovascular health. These contractions can manifest as premature atrial contractions (PACs) or premature ventricular contractions (PVCs). In your case, given your history of mitral valve prolapse, it is important to understand the implications of these symptoms.
Symptoms of Early Heart Contractions
1. Skipped Beats: Many patients describe the sensation of a "skipped" or "missed" heartbeat, which is often how early contractions are perceived. This is typically due to the heart's electrical system firing earlier than expected, leading to a feeling of irregularity.
2. Palpitations: As you described, the feeling of your heart "pounding" or "thumping" can occur after a premature beat. This sensation is often more pronounced after a PAC or PVC, as the heart may contract forcefully to compensate for the earlier beat.
3. Coughing or Breathing Changes: Some individuals report that certain actions, like coughing or changing positions, can trigger or alter the sensation of palpitations. This can be due to changes in thoracic pressure affecting heart rhythm.
Risks Associated with Early Heart Contractions
In most cases, isolated premature contractions are benign, especially in individuals without underlying heart disease. However, your history of mitral valve prolapse and the recent changes in your symptoms warrant a closer look:
1. Mitral Valve Prolapse (MVP): This condition can be associated with arrhythmias, including PACs and PVCs. While MVP itself is often benign, it can sometimes lead to more significant arrhythmias, particularly if there are structural changes in the heart.
2. Increased Frequency of Symptoms: If you notice an increase in the frequency of these early contractions, or if they are accompanied by other symptoms such as dizziness, shortness of breath, or chest pain, it is essential to seek medical evaluation.
3. Potential for Atrial Fibrillation: Your concern about atrial fibrillation (AF) is valid. While PACs can be a normal finding, they can occasionally trigger AF, especially in individuals with predisposing factors. AF can lead to more serious complications, including stroke, if not managed appropriately.
Recommendations
1. Monitoring: Keep a diary of your symptoms, noting when they occur, their duration, and any associated activities. This information can be valuable for your healthcare provider.
2. Consultation with a Cardiologist: Given your history and the recent changes in your symptoms, it would be prudent to consult a cardiologist. They may recommend further evaluation, such as an electrocardiogram (ECG) or a Holter monitor, to assess the frequency and nature of your premature contractions.
3. Lifestyle Modifications: Reducing caffeine intake, managing stress, and ensuring adequate hydration can help minimize the occurrence of premature beats.
4. Medication Review: If you are on any medications, discuss with your doctor whether they could be contributing to your symptoms.
In conclusion, while early heart contractions are often benign, your specific symptoms and medical history necessitate a thorough evaluation to rule out any underlying issues. Early detection and management can help ensure your cardiovascular health remains stable. Always prioritize open communication with your healthcare provider regarding any changes in your symptoms.
