Erectile dysfunction or inflammation of the genital organs?
Hello Dr.
Du, I have been experiencing erectile dysfunction for about the past month.
When attempting to achieve an erection, it does not become fully rigid, appearing semi-rigid and semi-soft, and it quickly loses firmness without external stimulation.
I have observed that after abstaining for 3 to 5 days, the semen volume has decreased, with the milky portion being less prominent and appearing more watery.
Additionally, I have noticed some non-liquefied gel-like particles.
Regarding morning erections, I still experience them almost daily, but they also seem to be semi-rigid and semi-soft during sleep.
In terms of physical sensations, I have a slight swelling and discomfort in the right scrotum (or testicle) when cycling or sitting, although it is not very pronounced.
To add, the discomfort is more noticeable right after ejaculation, accompanied by slight swelling and pain in the glans.
I have already done some research to rule out testicular torsion (as there is no severe pain), but I am unsure if there might be other inflammatory conditions (such as epididymitis, scrotal inflammation, or prostatitis), or if it is simply erectile dysfunction.
I have scheduled an appointment with the urology department at the end of the month, but since these symptoms have persisted for over a month (initially I wanted to rest and observe), I am concerned that delaying treatment might lead to complications.
Therefore, I am seeking your preliminary assessment.
P.S.
My overall health is currently good, and my BMI is around 21.

Simple erectile dysfunction should not have significant delays in treatment.
Whether it is due to inflammation or simple erectile dysfunction, it is necessary to visit an outpatient clinic for evaluation.
Based on the information provided, psychogenic erectile dysfunction seems to be the most likely cause.

Erectile dysfunction (ED) can be a complex condition influenced by various physical and psychological factors. Based on your description, it seems you are experiencing a combination of symptoms that may suggest an underlying issue, possibly related to inflammation or other medical conditions affecting the genital area.
Firstly, the inability to achieve a full erection, particularly if it is characterized by a semi-rigid state, can be indicative of several potential causes. These may include vascular issues, hormonal imbalances, psychological factors, or even neurological conditions. Given that you mentioned experiencing this for about a month, it is essential to consider both physical and psychological aspects. The fact that you still experience morning erections suggests that your erectile tissue may still be functional, which is a positive sign.
The changes in your semen, such as decreased volume and the presence of non-liquefied gel-like particles, could indicate a potential issue with your prostate or seminal vesicles. Conditions like prostatitis (inflammation of the prostate) or seminal vesiculitis could lead to changes in semen characteristics and may also contribute to discomfort in the genital area. The sensation of swelling or pain in the right scrotum or testicle, especially during activities like cycling or sitting, could also point towards conditions such as epididymitis (inflammation of the epididymis) or other forms of scrotal inflammation.
It is crucial to note that while you have ruled out testicular torsion, which is a surgical emergency, other inflammatory conditions can still be serious and warrant medical attention. The discomfort you feel, particularly after ejaculation, could be a sign of inflammation or infection in the reproductive system.
Your plan to consult a urologist is a wise decision. They will likely perform a thorough examination, which may include a physical exam, ultrasound of the scrotum, and possibly some laboratory tests to assess for infections or other abnormalities. It is essential to communicate all your symptoms clearly to your healthcare provider, including the duration and nature of your symptoms, as well as any other relevant health history.
In the meantime, consider some lifestyle modifications that may help alleviate some symptoms. Avoiding activities that exacerbate discomfort, such as prolonged sitting or cycling, may be beneficial. Staying hydrated and maintaining a healthy diet can also support overall reproductive health.
In summary, while erectile dysfunction can stem from various causes, the additional symptoms you are experiencing suggest that an inflammatory process may be at play. It is crucial to pursue a thorough evaluation by a healthcare professional to determine the underlying cause and appropriate treatment. Early intervention can often lead to better outcomes, so your proactive approach in seeking medical advice is commendable.
