Pregnancy test strip issues?

I would like to ask the doctor: If a pregnancy test is taken more than 45 days after sexual intercourse, can pregnancy be almost ruled out? I conducted a home pregnancy test, but I read online that there are cases where a pregnancy of two to three months does not show positive results on a pregnancy test (using a home pregnancy test).
Is this possible? I would like to know the doctor's opinion.

Chen, 20~29 year old female. Ask Date: 2019/11/22

If a pregnancy test is taken more than 45 days after sexual intercourse, can pregnancy be almost ruled out? Answer: Generally, this is true for home pregnancy tests, but I have seen online reports of pregnancies that were already two to three months along where home pregnancy tests did not detect the pregnancy.
Is this possible? Answer: Be cautious with information found online.

When it comes to home pregnancy tests (HPTs), their reliability increases significantly as time passes after potential conception. If you are testing 45 days after unprotected intercourse, the likelihood of getting an accurate result is quite high, especially if your menstrual period has not arrived. Most home pregnancy tests are designed to detect the hormone human chorionic gonadotropin (hCG), which is produced shortly after a fertilized egg attaches to the uterine lining.
Typically, hCG levels can be detected in urine about 10 to 14 days after conception, which corresponds to the time of a missed period. Therefore, by 45 days, if you have not had your period and the test is negative, it is reasonable to conclude that you are unlikely to be pregnant. However, there are exceptions to this rule.

One potential reason for a false negative result, even after 45 days, could be related to the sensitivity of the test used. Some tests are more sensitive than others and can detect lower levels of hCG. If you used a less sensitive test or if the test was not performed correctly (for example, using diluted urine), it could yield a negative result despite the presence of pregnancy.
Another factor to consider is the possibility of a biochemical pregnancy, where an embryo implants but does not develop properly, leading to a very early miscarriage. In such cases, hCG levels may rise briefly before falling again, which could lead to confusion when testing.

Additionally, certain medical conditions can affect hCG production or the menstrual cycle, such as polycystic ovary syndrome (PCOS) or other hormonal imbalances. These conditions can lead to irregular periods and may complicate the interpretation of pregnancy tests.

Regarding the concern about being pregnant for two to three months without detection, while it is rare, it is not impossible. Factors such as ectopic pregnancies, where the embryo implants outside the uterus, can sometimes lead to delayed hCG production. However, in most cases, an ectopic pregnancy would still produce enough hCG to be detected by a home pregnancy test.

In conclusion, if you are 45 days past potential conception and have received a negative result on a home pregnancy test, it is highly likely that you are not pregnant. However, if you have concerns or if your menstrual cycle remains irregular, it is advisable to consult a healthcare professional for further evaluation. They may recommend a blood test, which is more sensitive and can provide a definitive answer regarding pregnancy status. Additionally, they can help address any underlying issues that may be affecting your menstrual cycle or overall reproductive health.
