Post-operative issues after wisdom tooth extraction?

Hello, I had my lower right wisdom tooth extracted 10 days ago and I still have stitches.
Yesterday, the extraction site suddenly became very painful, radiating pain to my throat and ear.
Today, the stitches on the side came out on their own, and there is a foul odor coming from my mouth.
However, I am scheduled to have the stitches removed next Monday.
I would like to ask the doctor: 1.
Is it concerning that the stitches have come out? Should I seek immediate attention or can I wait until the scheduled stitch removal? 2.
Is the foul odor coming from an infection at the extraction site? If so, are there any methods to address this? Thank you, doctor.

Hello, 1.
There may be bacteria or food residue in the wound causing infection, leading to inflammation and pain.
It is recommended to return for a check-up immediately.
2.
Try to keep your mouth clean; consider using mouthwash to help disinfect and kill bacteria, but it is still advisable to return for a check-up with your physician.
Thank you.

Hello! It sounds like you're experiencing some discomfort after your wisdom tooth extraction, and I understand how concerning that can be. Let’s address your questions one by one.

1. Concern About the Stitches: The fact that your stitches have come out early can be a concern, especially if the extraction site is still healing. Generally, stitches are used to help close the wound and promote healing. If they come out prematurely, it may expose the underlying tissue, which can lead to complications such as delayed healing or infection. However, if the area is not excessively bleeding and you are not experiencing severe pain, it might not be an immediate emergency. It is advisable to contact your dentist or oral surgeon to discuss the situation. They may want to see you sooner than your scheduled appointment to ensure that everything is healing properly.

2. Bad Breath and Possible Infection: The foul odor you are experiencing could indeed be a sign of infection, especially if it is accompanied by pain radiating to your throat and ear. After a tooth extraction, it is not uncommon for some degree of discomfort and bad breath to occur, but if the smell is particularly strong or offensive, it could indicate that food particles are trapped in the extraction site, leading to bacterial growth. This can result in a condition known as "dry socket" or an infection of the extraction site (alveolar osteitis).
To manage this situation until you can see your dentist, here are a few steps you can take:
- Maintain Oral Hygiene: Gently rinse your mouth with warm salt water several times a day, especially after eating. This can help keep the area clean and reduce bacteria.

- Avoid Irritants: Stay away from smoking, alcohol, and spicy foods, as these can irritate the extraction site and worsen inflammation.

- Pain Management: Over-the-counter pain relievers like ibuprofen can help manage pain and reduce inflammation. However, make sure to follow the dosage instructions on the package.

- Stay Hydrated: Drink plenty of fluids to help flush out any food particles and keep your mouth moist.

- Monitor Symptoms: Keep an eye on your symptoms. If you notice increased swelling, persistent pain, or fever, it’s important to seek medical attention sooner rather than later.

In summary, while it may be possible to wait until your scheduled appointment to have the stitches checked, it’s best to consult with your dentist about the early loss of stitches and the bad breath. They can provide you with the most accurate advice based on a thorough examination. If you suspect an infection, it’s crucial to address it promptly to avoid further complications. Take care, and I hope you feel better soon!
