Semen issues
Doctor, I am 31 years old and have been experiencing some issues for the past six months.
I suddenly noticed a change from firm erections to semi-erections, and my semen has been watery and yellowish, resembling water.
This has been going on for almost a year now.
It doesn't have the thick, milky white consistency it used to have.
I have had urine and testosterone tests done, both of which came back normal, but I have not yet tested my semen.
Could this indicate a problem with my testicles or something else? Thank you for your help.
Additionally, I used to have no trouble achieving an erection during intercourse, but now I have completely lost my libido and cannot maintain an erection unless I use my hand, and even then it becomes soft again shortly after.
I am unsure what the issue could be.
Thank you for addressing these two concerns.

Hello, generally speaking, the color of semen is determined by its composition.
Normal semen is grayish-white or slightly yellowish.
Sometimes, abstinence can cause the color to appear more yellow, which is considered normal.
However, if you notice that the semen is yellow and have ruled out prolonged abstinence as a factor, further examination is necessary, such as testing for reproductive tract infections.
As for factors affecting penile rigidity, there are many possible influences, including psychological stress, dietary improvements, smoking, and alcohol consumption.
It is advisable to maintain a regular routine and relax.
If there is no improvement, seeking medical treatment is recommended.

Changes in semen consistency and hardness, as well as issues with erectile function and libido, can be concerning and may stem from a variety of underlying causes. Given your age of 31 and the symptoms you've described, it's important to explore both physiological and psychological factors that could be contributing to these changes.
Semen Consistency and Hardness
1. Semen Composition: Normal semen is typically a thick, whitish fluid that contains sperm and various other components, including enzymes, proteins, and sugars. Changes in the consistency of semen, such as becoming more watery or yellowish, can indicate several potential issues. This may include hormonal imbalances, infections, or other medical conditions.
2. Hormonal Factors: Testosterone plays a crucial role in the production of semen and maintaining sexual function. Although you mentioned that your testosterone levels were normal, it’s worth noting that fluctuations can occur. Additionally, other hormones, such as luteinizing hormone (LH) and follicle-stimulating hormone (FSH), should also be evaluated, as they regulate testosterone production and sperm development.
3. Infections or Inflammation: Conditions such as prostatitis (inflammation of the prostate) or sexually transmitted infections (STIs) can affect semen quality. These conditions may not always present with obvious symptoms, so a thorough examination, including a semen analysis, is recommended to rule out any infections.
4. Lifestyle Factors: Diet, hydration, and lifestyle choices (such as smoking, alcohol consumption, and drug use) can significantly impact semen quality. A diet lacking in essential nutrients or high in processed foods may lead to changes in semen consistency.
Erectile Dysfunction and Libido
1. Psychological Factors: Stress, anxiety, and depression can greatly affect sexual desire and erectile function. If you are experiencing significant life changes or stressors, it may be beneficial to address these psychological aspects, possibly with the help of a mental health professional.
2. Physical Health: Conditions such as diabetes, hypertension, and cardiovascular disease can lead to erectile dysfunction. Even if you feel healthy, it’s important to consider that underlying health issues may not always be apparent. A comprehensive health check-up, including cardiovascular assessments, may be warranted.
3. Medications: Certain medications can have side effects that impact libido and erectile function. If you are taking any medications, review them with your healthcare provider to determine if they could be contributing to your symptoms.
4. Age-Related Changes: While 31 is relatively young, some men may begin to experience changes in sexual function earlier than others. This can be due to a combination of physical and psychological factors.
Recommendations
1. Semen Analysis: Since you have not yet had a semen analysis, this should be a priority. It can provide valuable information about sperm count, motility, and morphology, as well as the overall health of your semen.
2. Consult a Urologist: A urologist specializes in male reproductive health and can conduct a thorough evaluation, including hormone testing and possibly imaging studies, to identify any underlying issues.
3. Lifestyle Modifications: Consider making lifestyle changes that promote overall health. This includes a balanced diet rich in fruits, vegetables, and whole grains, regular exercise, adequate hydration, and minimizing alcohol and tobacco use.
4. Mental Health Support: If you suspect that psychological factors may be at play, seeking support from a therapist or counselor can be beneficial. They can help you work through any stress or anxiety that may be affecting your sexual health.
5. Open Communication: If you are in a relationship, discussing these changes with your partner can help alleviate some of the stress and anxiety surrounding sexual performance.
In conclusion, changes in semen consistency and erectile function can be multifactorial. A thorough evaluation by a healthcare professional, including a semen analysis and possibly further hormonal testing, is essential to identify the underlying causes and develop an appropriate treatment plan.
