Post-operative issues after the extraction of four wisdom teeth (the two mentioned are horizontal wisdom teeth)?

Hello, I had my last two wisdom teeth extracted on December 6th, including the upper left normal wisdom tooth and the lower left horizontal wisdom tooth (which underwent regenerative surgery).
My recovery is going well, except for the stitches from the upper right wisdom tooth that have been removed, while the others remain.
There is still a small hard area on the left cheek that hasn’t fully swollen down.
Recently, I noticed some asymmetry in my face when looking in the mirror.
Will the asymmetry resolve completely after the swelling goes down and the stitches are removed? Thank you very much.

Ms. Hsieh, 20~29 year old female. Ask Date: 2019/12/19

Dr. Huang Tingfang reply Dentistry

Dear Ms.
Hsieh,
Hello! Based on your description, it appears to be a hematoma following the extraction of the lower left wisdom tooth.
The swelling should subside within a week.
If you have any issues, please seek medical attention as soon as possible.
Take care! Wishing you all the best and a smooth recovery!
Sincerely,
Dr.

After wisdom tooth extraction, it is not uncommon to experience some degree of facial asymmetry, particularly in the initial healing stages. This can be attributed to several factors, including swelling, bruising, and the healing process itself. In your case, since you had both a regular and a horizontally impacted wisdom tooth removed, the surgical trauma and subsequent healing may contribute to the temporary asymmetry you are observing.

Understanding Facial Asymmetry Post-Extraction
1. Swelling and Inflammation: After surgery, the body responds with inflammation, which can lead to swelling around the extraction sites. This swelling can cause one side of your face to appear larger or different in shape compared to the other. Typically, this swelling peaks within the first 48 to 72 hours post-surgery and gradually subsides over the following days to weeks.

2. Healing Process: The healing process involves the formation of scar tissue, which can also contribute to temporary asymmetry. As the tissues heal and the swelling diminishes, you should see a gradual return to a more symmetrical appearance.

3. Muscle Coordination: Post-operative discomfort and swelling can affect how your facial muscles function. If you are experiencing any tightness or discomfort on one side of your face, it may lead to altered muscle movement, contributing to the perception of asymmetry.

Will Your Face Return to Symmetry?
In most cases, once the swelling subsides and the stitches are removed, facial symmetry should improve significantly. It is essential to give your body time to heal. The hard area you mentioned on the left side of your face may be related to residual swelling or the formation of scar tissue, which should resolve over time.

Recommendations for Recovery
1. Follow-Up Care: Make sure to attend any follow-up appointments with your dentist or oral surgeon. They can assess your healing progress and address any concerns you may have regarding asymmetry or other symptoms.

2. Ice and Heat Therapy: In the initial days post-surgery, applying ice packs can help reduce swelling. After a few days, switching to warm compresses may promote blood flow and aid in healing.

3. Gentle Massage: Once the initial swelling has decreased, gentle massage around the area can help improve circulation and may assist in reducing any tightness or discomfort.

4. Monitor Symptoms: Keep an eye on any unusual symptoms, such as persistent swelling, pain, or changes in sensation. If you notice any concerning signs, do not hesitate to reach out to your healthcare provider.

5. Patience: Healing takes time, and it is important to be patient with your body as it recovers. Most patients see significant improvement in facial symmetry within a few weeks post-extraction.

Conclusion
In summary, while it is normal to experience some asymmetry after wisdom tooth extraction, most individuals see a return to symmetry as they heal. If you have concerns about your recovery or if the asymmetry persists beyond a reasonable timeframe, consulting with your oral surgeon or a specialist in facial aesthetics may provide additional reassurance and guidance. Remember, your body is undergoing a healing process, and with time, you should see improvements.

May I ask how old you are this year? If the condition has persisted for more than five years, it is almost impossible to treat. As for cosmetic surgery, it depends on the severity of the condition and whether the procedure will actually lead to improvement.
