Hello, Dr.
Du.
Today I noticed four small white bumps, about 1mm in size, on both sides of my frenulum.
They are not painful or itchy.
Here is the image link: https://upload.cc/i1/2020/02/03/bgdwcL.jpg.
Could you please advise whether this is more likely to be frenular papules, genital herpes, condyloma, or another sexually transmitted infection? If it is frenular papules, what treatment options are available to reduce their appearance for cosmetic reasons? Thank you for your assistance, Dr.
Du.

Hello: Genital herpes can rupture and produce fluid, while genital warts can grow, with some growing quickly and others slowly.
If they increase in number, it is advisable to seek medical attention (especially if there has been unprotected sexual intercourse).
Pearly penile papules are more stable and do not increase or decrease in number.
In fact, genital warts and pearly penile papules have similarities.
If you have had sexual intercourse, it is recommended to consult a urologist.
Wishing you good health.
P.S.
Many people have pearly penile papules.

More Info

The presence of white bumps on the frenulum, as you described, can be concerning and may lead to questions about their nature and potential treatment options. Based on your description of the bumps being approximately 1mm in size, painless, and non-itchy, they could likely be classified as Fordyce spots or mucous cysts, which are common and benign conditions. However, it is essential to differentiate these from other potential causes, such as genital herpes, warts (caused by the human papillomavirus), or other sexually transmitted infections.

Fordyce Spots: These are small, painless, pale bumps that are actually sebaceous glands without hair follicles. They are completely harmless and do not require treatment unless for cosmetic reasons.
Mucous Cysts: These can occur on the frenulum and are typically fluid-filled sacs that can appear white or translucent. They are also benign and may resolve on their own.

Genital Warts: Caused by certain strains of HPV, these can appear as small, flesh-colored or white bumps and may be itchy or painful. They often require treatment if diagnosed.

Genital Herpes: This condition is characterized by painful blisters or sores, which does not match your description of the bumps being painless and non-itchy.

Given the characteristics you've described, it seems less likely that these bumps are related to a sexually transmitted infection, especially if they are not causing any discomfort. However, it is crucial to consult a healthcare professional for an accurate diagnosis, as visual examination and possibly further testing may be necessary.

Treatment Options
If the bumps are indeed Fordyce spots or mucous cysts and you are concerned about their appearance, there are a few options you might consider:
1. Laser Therapy: This is a cosmetic procedure that can effectively reduce the appearance of Fordyce spots. It involves using a laser to target and remove the spots without damaging surrounding tissue.

2. Cryotherapy: This method involves freezing the spots with liquid nitrogen, which can help in reducing their visibility.

3. Electrosurgery: This technique uses electrical currents to remove the spots. It is effective but may require local anesthesia.

4. Topical Treatments: While there are no specific topical treatments for Fordyce spots, some people use retinoid creams to promote skin turnover, which may help in reducing their appearance over time.

5. Consultation with a Dermatologist: If you are concerned about the cosmetic aspect or if the bumps change in appearance, a consultation with a dermatologist can provide tailored advice and treatment options.

Conclusion
In summary, the white bumps on your frenulum are likely benign, but it is essential to get a professional evaluation to rule out any serious conditions. If they are determined to be Fordyce spots or mucous cysts, various cosmetic treatments are available to address any aesthetic concerns. Always prioritize consulting with a healthcare provider for an accurate diagnosis and appropriate management.
