Inquiry about methods for cleaning smegma?
Hello, Dr.
Du.
I recently visited a urologist who told me that the small particles I have are not a sexually transmitted infection.
However, he advised me to clean under the foreskin properly.
At the time, I forgot to ask the doctor how to do this appropriately.
When I shower, I have tried rinsing with warm water, but it feels a bit sensitive, and I can't seem to wash away the smegma completely.
I just tried using a clean cotton swab dipped in iodine to remove some of it, and I did manage to get quite a bit off, but there are still some yellowish-white residues on the surface.
I would like to ask the doctor if using iodine for cleaning is appropriate, or if it is not recommended, as it also causes a sensitive, stinging sensation.
Could you please advise me on the proper method and whether it is necessary to clean until there is nothing visible, or if a proper cleaning is sufficient? I am worried that over-cleaning might cause skin damage and lead to an infection without me realizing it.

Hello: Your condition should not be treated with Betadine as it may cause irritation.
Instead, rinse with clean water or gently scrape with a cotton swab that is very wet.
Of course, the foreskin should be retracted, and do this a little each day.
Do not apply too much at once, as it may cause injury.
After some time, the entire foreskin should be exposed, which is normal.
After retracting the foreskin, you can refer to the following attachment for guidance.
Wishing you good health.

When it comes to cleaning under the foreskin, it is essential to approach the task with care and an understanding of proper hygiene practices. The foreskin can accumulate smegma, a combination of dead skin cells, oils, and moisture, which can lead to irritation or infection if not cleaned regularly. Here are some effective methods and considerations for cleaning under the foreskin:
1. Gentle Washing with Warm Water: The most recommended method for cleaning under the foreskin is to use warm water. During your shower, gently retract the foreskin and rinse the area with warm water. This should be done carefully to avoid any discomfort or irritation. It’s important to note that using soap in this area can sometimes lead to irritation, so it’s best to stick with water unless otherwise advised by a healthcare professional.
2. Avoiding Harsh Chemicals: While you mentioned using iodine, it is generally not recommended to use antiseptics like iodine or alcohol for cleaning sensitive areas such as under the foreskin. These substances can cause irritation and may lead to a burning sensation, as you experienced. Instead, focus on gentle cleaning methods that do not disrupt the natural balance of the skin.
3. Use of Cotton Swabs: If you find that there is still smegma present after rinsing, you might consider using a clean cotton swab to gently remove any remaining buildup. However, be very cautious not to apply too much pressure or to insert the swab too deeply, as this could cause injury or irritation.
4. Frequency of Cleaning: It is not necessary to clean under the foreskin to the point where there is absolutely no visible smegma. Regular cleaning, such as during showers, should suffice. The goal is to maintain hygiene and prevent any buildup that could lead to irritation or infection. If you notice any persistent irritation, redness, or unusual discharge, it is advisable to consult a healthcare professional.
5. Signs of Infection: Be aware of signs of infection, which may include increased sensitivity, redness, swelling, or discharge that has an unusual odor. If you experience any of these symptoms, it is crucial to seek medical advice promptly.
6. Consulting a Healthcare Professional: If you have ongoing concerns about cleaning under the foreskin or if you experience discomfort, it is best to consult with a urologist or a healthcare provider. They can provide personalized advice based on your specific situation and ensure that you are following the best practices for hygiene.
In summary, the best approach to cleaning under the foreskin is to use warm water and gentle techniques. Avoid harsh chemicals like iodine or alcohol, and be mindful of any signs of irritation or infection. Regular cleaning during bathing is usually sufficient, and if you have any concerns, do not hesitate to reach out to a healthcare professional for guidance.
