Eye Strain: Causes and When to Seek Medical Advice - Ophthalmology

Medical consultation

Hello doctor, I would like to ask you about a sensation I've been experiencing this month in my left eye.
It feels like there is a pulling sensation inside, as if someone is tugging on my eyeball.
This happens several times a day but doesn't last long.
I went to the hospital for an examination, and the doctor said my eyes are too fatigued and prescribed me eye drops for fatigue.
I have myopia of -7.00 and astigmatism of -1.75 in both eyes.
Yesterday, I experienced this sensation again, and suddenly several bright spots appeared in my vision, which disappeared after about ten seconds.
What could be causing this? Do I need to go to the hospital for further examination?

Dai Dai, 20~29 year old female. Ask Date: 2020/03/05

Hello! It sounds like you are experiencing some concerning symptoms with your left eye, including a pulling sensation and the appearance of multiple light spots. These symptoms can be quite distressing, and it's important to understand their potential causes and when to seek medical advice.

Firstly, the pulling sensation you describe could be related to several factors. Eye strain, also known as asthenopia, is a common issue, especially for individuals who spend long hours focusing on screens or reading. This strain can lead to discomfort, fatigue, and even visual disturbances. The fact that your doctor prescribed eye drops for fatigue suggests that they believe your symptoms may be related to overuse or strain on your eyes.

The appearance of light spots, or photopsia, can be caused by various factors, including:
1. Vitreous Detachment: As we age, the vitreous gel inside the eye can shrink and pull away from the retina, leading to flashes of light or spots. This is often harmless but can sometimes indicate a more serious condition.

2. Retinal Issues: Conditions such as retinal tears or detachments can also cause flashes of light or spots in your vision. These are more serious and require immediate medical attention.

3. Migraine Aura: Some individuals experience visual disturbances, including light spots, as part of a migraine aura, even without a headache.

4. Ocular Migraines: Similar to migraine aura, these can cause temporary visual disturbances, including light spots, and are often not accompanied by pain.

Given your symptoms, especially the combination of the pulling sensation and the appearance of light spots, it is advisable to seek further medical evaluation. While your doctor may have initially attributed your symptoms to eye strain, the sudden appearance of light spots warrants a more thorough examination to rule out any serious conditions, particularly those affecting the retina.

When to Seek Medical Advice
You should consider seeking immediate medical attention if you experience any of the following:
- Sudden changes in vision: If you notice a sudden increase in the number of light spots, flashes, or any other changes in your vision.

- Persistent symptoms: If the pulling sensation or light spots continue or worsen over time.

- Accompanied symptoms: If you experience other symptoms such as pain, redness, or swelling in the eye, or if you have a headache that accompanies the visual disturbances.

Recommendations
1. Follow Up with an Eye Specialist: If your symptoms persist or worsen, it’s essential to see an ophthalmologist for a comprehensive eye exam. They can perform tests to check the health of your retina and other structures in the eye.

2. Limit Screen Time: If you suspect eye strain, try to reduce your screen time and take regular breaks using the 20-20-20 rule: every 20 minutes, look at something 20 feet away for at least 20 seconds.

3. Use Proper Lighting: Ensure that your workspace is well-lit to reduce strain on your eyes.

4. Stay Hydrated: Dehydration can contribute to eye strain, so make sure you drink enough water throughout the day.

5. Consider Vision Correction: If you are not already wearing corrective lenses, or if your prescription has changed, it may be beneficial to have your eyes checked for an updated prescription.

In conclusion, while eye strain is a common issue, the symptoms you are experiencing, particularly the light spots, should not be ignored. Seeking a thorough evaluation from an eye care professional will help ensure that any potential issues are addressed promptly. Your eye health is important, and taking proactive steps can help maintain your vision and overall well-being.
