Ejaculatory pain and related issues?
Hello, doctor.
I have an issue with excessive foreskin.
During masturbation, I can push it back with my hand, but when I pull it back and forth, the skin tends to get caught on the glans after an erection.
However, I always pull it back during ejaculation for easier cleaning.
I've noticed that after ejaculation, it feels very pleasurable, but right before finishing, I experience a slight pain that lasts about one or two seconds, which seems to originate from the area near the frenulum of the foreskin.
I would like to know the exact cause of this.
Previously, I wasn't very skilled at cleaning smegma, so I often felt sticky and noticed a thin layer of white substance on it, which caused me a lot of psychological stress.
However, since I started exposing the glans and ensuring I dry it properly after cleaning, this issue has disappeared.
Additionally, I experienced pain in the groin during middle school, where my testicles hurt, but it resolved after a night's sleep, and I didn't tell my family.
In high school, I had a similar episode, and when I went to the hospital, the doctor diagnosed it as a varicocele.
After doing some inversions and soaking in hot water at home, it improved significantly.
I also carefully observe my semen after masturbation.
Although it appears white (sometimes slightly yellow), when I wipe with tissue, the thin semen on the tissue has a somewhat yellow tint.
I asked a doctor about this, and he said as long as it's not red, I shouldn't worry too much.
However, the psychological burden remains, and I find it hard not to think about it, even though I try.
I hope you can provide some advice to help improve my psychological issues.

Ejaculation pain, or pain during ejaculation, can be a distressing symptom for many individuals. In your case, the discomfort you experience seems to be associated with the retraction of the foreskin during ejaculation, which may be causing some friction or irritation. This is not uncommon, especially in individuals with a longer foreskin or those who have not been circumcised.
The pain you describe occurring at the moment of ejaculation, particularly near the frenulum (the sensitive band of tissue on the underside of the penis), could be due to several factors:
1. Friction and Irritation: If the foreskin is not adequately lubricated or if there is excessive friction during masturbation, it can lead to temporary pain or discomfort. This is especially true if the foreskin is pulled back during ejaculation, as the sudden movement can cause irritation.
2. Infection or Inflammation: Although you mentioned that you have improved your hygiene and the issue of smegma (the white substance) has resolved, any residual inflammation or irritation from previous infections could still lead to discomfort. Conditions like balanitis (inflammation of the glans) can cause pain during ejaculation.
3. Psychological Factors: Anxiety and stress can amplify physical sensations, including pain. If you are preoccupied with the fear of pain or discomfort, it can create a cycle of anxiety that makes the experience worse.
4. Underlying Conditions: While less common, conditions such as prostatitis (inflammation of the prostate) or other urological issues could also contribute to pain during ejaculation.
Regarding your previous experiences with testicular pain and the diagnosis of varicocele, it’s good to know that those issues have resolved. Varicocele can sometimes lead to discomfort, but it typically does not cause pain during ejaculation.
As for the color of your semen, variations in color can occur and are often normal. Yellowish tints can be due to several factors, including diet, hydration levels, or the presence of certain substances in the semen. As long as there is no blood (red coloration) or other concerning symptoms, this is generally not a cause for alarm.
Recommendations:
1. Hygiene: Continue to maintain good hygiene practices. Regular cleaning of the genital area can help prevent irritation and infections.
2. Lubrication: If friction is a concern during masturbation, consider using a water-based lubricant to reduce irritation.
3. Consult a Urologist: If the pain persists or worsens, it would be wise to consult a urologist. They can perform a thorough examination and rule out any underlying conditions that may require treatment.
4. Psychological Support: If anxiety about your symptoms is affecting your quality of life, consider speaking with a mental health professional. Cognitive-behavioral therapy (CBT) can be particularly effective in managing anxiety and stress related to health concerns.
5. Education and Reassurance: Sometimes, understanding that variations in bodily functions are normal can alleviate anxiety. Educating yourself about male reproductive health can help reduce fears and misconceptions.
In summary, while occasional pain during ejaculation can be normal, persistent or severe pain should be evaluated by a healthcare professional. Addressing both the physical and psychological aspects of your concerns will be crucial in managing your symptoms effectively.
