Size of the glans during erection?
Hello doctor, as mentioned in the subject, I have recently noticed that sometimes my erect penis appears quite large, but the glans seems small.
Other times, the erect glans feels like it is of normal size.
I have phimosis, and during an erection, the glans does not become exposed, although I can manually retract the foreskin to reveal it.
I am unsure if this is the cause or if there might be other underlying health risks involved.
Thank you.

Hello: There shouldn't be any other issues.
I personally believe it may be related to congestion, which could slightly affect size.
Wishing you good health.

Understanding variations in penis and glans size during erection can be a common concern among men, and it is important to approach this topic with both anatomical knowledge and an understanding of individual variations.
Firstly, it is essential to recognize that the penis is composed of erectile tissue that engorges with blood during sexual arousal, leading to an erection. The penis consists of three main parts: the shaft, the glans (or head), and the surrounding skin, which includes the foreskin in uncircumcised men. The size of the penis and glans can vary significantly among individuals and can also change during different states of arousal or erection.
The phenomenon you are experiencing, where the penis appears larger while the glans seems smaller at times, can be attributed to several factors:
1. Erectile Tissue Dynamics: The penis has two corpora cavernosa and one corpus spongiosum, which fill with blood during an erection. The degree of engorgement can vary, leading to differences in the perceived size of the shaft versus the glans. If the shaft engorges more than the glans, it may create the illusion of a smaller glans.
2. Foreskin and Glans Exposure: Since you mentioned having a long foreskin that does not retract easily during an erection, this could contribute to the perception of a smaller glans. The foreskin can cover the glans, making it appear less prominent, especially if it does not retract fully during arousal. This is a common situation for men with phimosis (tight foreskin), which can affect how the glans is visually perceived.
3. Variability in Arousal: The level of arousal can also affect the size of the glans. Factors such as psychological state, physical stimulation, and overall health can influence blood flow and engorgement, leading to variations in size during different instances of arousal.
4. Hydration and Temperature: Environmental factors such as temperature and hydration levels can also affect blood flow and, consequently, the size of the penis and glans. For example, cold temperatures can lead to constriction of blood vessels, resulting in a smaller appearance.
5. Health Considerations: While variations in size during erection are typically normal, it is essential to be aware of any accompanying symptoms that might indicate an underlying health issue. If you experience pain, discomfort, or significant changes in erectile function, it would be prudent to consult a healthcare professional for a thorough evaluation.
In summary, the variations in penis and glans size during erection that you are experiencing can be attributed to a combination of anatomical factors, the dynamics of erectile tissue, and the influence of the foreskin. If you have concerns about your foreskin or any potential underlying conditions, it may be beneficial to consult with a urologist. They can provide personalized advice and discuss options for treatment if necessary, such as circumcision or other interventions to address phimosis. Remember, open communication with a healthcare provider is key to addressing any concerns regarding sexual health and anatomy.
