Sudden sharp pain in the right breast?

Hello doctor, this morning I woke up with a sudden sharp pain in my right breast (below the nipple).
I would like to know if this is normal.
My period is due in 4.5 days, and I usually experience breast tenderness before my period.
Recently, my nipple has been a bit dry, and I have been applying lotion.
Additionally, I had surgery for a papilloma on the outer side of my right breast in the summer of 2018, and the results were benign.
I had an ultrasound check earlier this month, and the results were normal.

Dr. Li Zhonghuai reply Surgery

Hello! There are many causes of breast pain, and often the exact reason cannot be identified.
Generally, after ruling out tumors, the focus is on symptomatic treatment.
You had a breast ultrasound at the beginning of the month, which has essentially ruled out any tumor issues.
Pain is typically managed with muscle relaxants; you can take them as needed when the pain occurs.
Take care! Changhua Hospital is concerned about you!

Experiencing sudden sharp pain in the breast can be concerning, especially if it occurs unexpectedly. In your case, the pain is localized to the area beneath the right nipple and coincides with your menstrual cycle, which is an important factor to consider. Breast pain, or mastalgia, can arise from various causes, and understanding these can help alleviate your concerns.

Firstly, hormonal fluctuations are a common cause of breast pain, particularly in relation to the menstrual cycle. Many women experience cyclical breast pain due to changes in estrogen and progesterone levels that occur before menstruation. This type of pain is often described as a heaviness or tenderness in the breasts and can sometimes present as sharp pain. Given that your period is approaching in a few days, it is plausible that the pain you are experiencing could be linked to these hormonal changes.

Additionally, you mentioned a history of a benign papilloma in the right breast, which was surgically removed in 2018. Since you have had a recent ultrasound that showed normal results, it is unlikely that the current pain is related to any serious underlying condition, such as breast cancer. Benign breast conditions, including cysts and fibrocystic changes, can also lead to discomfort and pain, particularly in relation to the menstrual cycle.

Another factor to consider is the possibility of musculoskeletal pain. Sometimes, pain in the breast area can be referred from the chest wall or muscles surrounding the breast. Activities such as exercise, poor posture, or even sleeping in an awkward position can lead to muscle strain, which may manifest as sharp pain in the breast area. If you have recently engaged in physical activities or noticed changes in your sleeping habits, this could be a contributing factor.

You also mentioned that your nipple has been feeling dry, which could indicate a need for better hydration or moisturization. While dryness itself is not typically associated with sharp pain, it can lead to irritation, which might exacerbate any discomfort you are feeling.

In summary, while sudden sharp pain in the breast can be alarming, it is often related to hormonal changes, benign breast conditions, or musculoskeletal issues. Given your recent normal ultrasound and the benign nature of your past breast condition, it is likely that the pain you are experiencing is not indicative of a serious problem. However, it is essential to continue monitoring your symptoms. If the pain persists, worsens, or is accompanied by other concerning symptoms (such as changes in breast appearance, discharge from the nipple, or new lumps), it would be prudent to consult with a healthcare professional for further evaluation.

In the meantime, you may find relief through supportive measures such as wearing a well-fitted bra, applying warm or cold compresses to the affected area, and considering over-the-counter pain relief if necessary. Keeping a symptom diary to track the timing and nature of the pain in relation to your menstrual cycle may also provide useful information for future consultations with your healthcare provider. Always prioritize your health and do not hesitate to seek medical advice if you feel uncertain about your symptoms.
