Glans fungal infection

Hello Dr.
Du,
Recently, I suddenly experienced dryness and wrinkling of the glans, with small spots (the color of the spots changes with the skin color).
The skin surface appears uneven and not smooth (especially noticeable during erection).
I often feel the urge to urinate, but there isn't always urine.
I later visited a doctor, who conducted a urinalysis that showed no infection.
After examining me, the doctor diagnosed a fungal infection and prescribed Clotrimazole Cream USP 1% to apply twice a day.
Since I am currently abroad and medical expenses are not cheap, I would like to ask you the following questions:
1.
I found online that I should not use soap for cleaning, but only wash with warm water and pat dry.
How long should I continue applying the antifungal cream twice a day before I can expect to heal? I have been applying it for almost 5 weeks; there has been improvement, but it is not completely healed.
2.
How can I tell if I have fully healed? Will the small spots on the glans, dryness, and wrinkling disappear?
3.
Recently, after ejaculation, I experience a sensation of needing to urinate, but there isn't always urine.
Is this related to the fungal infection? For your information, my recent sexual activity has been self-stimulation.
The urge to urinate is not as strong as before treatment, but there is still some discomfort, which usually disappears after I wake up the next day.
Previously, I would ejaculate once a day at most; now it is up to once every two days.
If I do not ejaculate, I do not experience this sensation.
If it is related to the fungal infection, will the urge to urinate after ejaculation cease once I am healed? If it is not a fungal issue, could it be related to the bladder, kidneys, or other organs?
I appreciate your assistance in answering these questions.
Thank you very much!

Hello: 1) If it hasn't improved after one week, pay attention to whether condoms were used during sexual intercourse, and make sure to dry off after urination.
2) Some conditions may just be like this; they may not cause redness or discharge.
3) It is not related to a fungal infection; it is recommended to urinate after sexual intercourse, maintain a normal lifestyle, exercise regularly, and drink plenty of water, which will help improve the situation.
Wishing you good health.

More Info

Fungal infections of the glans penis, commonly referred to as candidiasis or yeast infections, can be quite uncomfortable and may present with symptoms such as dryness, irritation, and changes in skin texture. In your case, it seems that you have been diagnosed with a fungal infection and prescribed Clotrimazole Cream USP 1%. Here’s a comprehensive overview of your concerns and some additional information regarding treatment and management of this condition.

1. Duration of Treatment: The duration of treatment with Clotrimazole Cream can vary based on the severity of the infection and individual response to the medication. Typically, antifungal creams like Clotrimazole are applied twice daily for at least two weeks. However, since you have been using it for almost five weeks with some improvement but no complete resolution, it may be advisable to consult your healthcare provider. They may recommend continuing the treatment for a bit longer or possibly switching to a different antifungal if necessary. It’s essential to follow the instructions provided by your healthcare provider and not to stop treatment prematurely, even if you notice some improvement.

2. Signs of Healing: You can determine if the infection is healing by observing the symptoms. Signs of improvement may include reduced redness, less irritation, and a gradual return to normal skin texture. The small spots you mentioned should diminish as the infection resolves. If the symptoms persist or worsen, it is crucial to seek further medical advice. A healthcare provider may perform a physical examination and possibly additional tests to confirm that the infection has cleared.

3. Post-Ejaculation Urgency: The sensation of urgency to urinate after ejaculation can be related to several factors. While it may be associated with the fungal infection, it could also indicate irritation of the urethra or bladder. If this sensation persists after the fungal infection has been treated, it may warrant further investigation. Conditions such as prostatitis or urinary tract infections (UTIs) could also lead to similar symptoms. It’s essential to monitor this symptom and discuss it with your healthcare provider if it continues.

4. Hygiene Practices: You mentioned that you have been advised to avoid soap and only use warm water for cleaning. This is generally good advice, as soaps can sometimes irritate the sensitive skin of the glans. After washing, ensure that the area is thoroughly dried, as moisture can promote fungal growth. Wearing loose-fitting, breathable underwear can also help reduce moisture and irritation.

5. Potential Complications: If left untreated, a fungal infection can lead to more severe symptoms or secondary infections. It’s also important to consider that recurrent infections may indicate an underlying issue, such as diabetes or an immune system deficiency. If you find that you are experiencing frequent recurrences of this condition, it would be wise to discuss this with your healthcare provider.

In conclusion, while fungal infections of the glans penis can be effectively treated with antifungal creams, it is crucial to follow the treatment regimen as prescribed and monitor your symptoms closely. If you have any concerns about the duration of treatment, the effectiveness of the medication, or any new symptoms that arise, do not hesitate to reach out to your healthcare provider for further evaluation and guidance. Maintaining good hygiene and being aware of any changes in your symptoms will aid in your recovery and overall health.
