Symptoms of glans inflammation?
Hello Doctor, I have been experiencing balanitis for over three months.
Starting from the second month, I noticed an increase in urinary frequency, going to the bathroom about 8 to 9 times during the day and 1 to 2 times at night.
Could this be primarily due to something other than balanitis, or could the balanitis have triggered other complications? Most of my urine is white, with occasional yellow urine, and there are no other significant abnormalities.
Previously, I was prescribed an ointment, which was initially effective, but the doctor did not specify how long to apply it, so I have been applying it intermittently.
Now, as soon as I stop the medication for a week after feeling normal, the symptoms return (itching and milky white discharge).
Currently, there is no noticeable odor when I retract the foreskin.
What tests do you recommend, and how should I explain my situation to the doctor?

Recurrent glans inflammation should prompt consideration of diabetes, especially if it is caused by prolonged foreskin coverage.
In such cases, circumcision may be considered.
Frequent urination is generally not directly related to glans inflammation.

Balanitis, or inflammation of the glans penis, can be a frustrating and uncomfortable condition, especially when it persists for an extended period, as you have experienced. Your symptoms, including frequent urination and the presence of a white discharge, suggest that there may be more than just balanitis at play.
Balanitis can be caused by various factors, including infections (bacterial, fungal, or viral), irritants (such as soaps or lotions), or underlying medical conditions (like diabetes). In your case, the fact that you have been experiencing symptoms for over three months raises the possibility of a chronic infection or an underlying condition that may be contributing to your symptoms.
The frequent urination you describe—8 to 9 times during the day and 1 to 2 times at night—could indicate a urinary tract infection (UTI) or another urinary issue. While balanitis itself can cause discomfort and irritation, it typically does not lead to significant changes in urinary frequency unless there is an associated infection or inflammation affecting the urinary tract. The white discharge you mentioned, along with occasional yellow urine, could also suggest a possible infection, particularly if the discharge is not typical for balanitis.
It is important to communicate your symptoms clearly to your healthcare provider. Here are some suggestions on how to approach your next visit:
1. Document Your Symptoms: Keep a detailed record of your symptoms, including the frequency of urination, the appearance of your urine, any discharge, and any other symptoms like itching or discomfort. This information will help your doctor understand the progression of your condition.
2. Discuss Treatment History: Explain the treatments you have tried, including the topical medication you were prescribed. Mention how long you used it, any improvements you noticed, and how quickly the symptoms returned after stopping the medication.
3. Inquire About Further Testing: Given the duration of your symptoms and the changes in urination, ask your doctor if further testing is warranted. This could include a urinalysis to check for signs of infection, a culture to identify any pathogens, or even a referral to a urologist for specialized evaluation.
4. Explore Underlying Conditions: Discuss the possibility of underlying conditions, such as diabetes, which can predispose individuals to infections and inflammation. If you have any risk factors for diabetes or other systemic conditions, it may be worth investigating.
5. Follow-Up Care: Ask about the appropriate duration for using topical treatments and whether there are alternative therapies that may be more effective for your condition. It’s crucial to have a clear treatment plan and understand when to follow up if symptoms persist.
In summary, while your balanitis may be the initial concern, the accompanying symptoms of frequent urination and discharge suggest that there could be additional factors at play. A thorough evaluation by your healthcare provider, including appropriate tests and discussions about your treatment history, will be essential in determining the underlying cause and developing an effective management plan. Remember, open communication with your doctor is key to finding relief and addressing any complications that may arise.
