Hemorrhoid stapling surgery
Hello, teacher.
Last time I asked about the risks of complications associated with a second circular stapled hemorrhoidopexy surgery.
I would like to understand what those risks are.
I am still experiencing rectal bleeding during bowel movements, with about 6 to 7 drops each time, and medication treatment has not improved the situation.
If I undergo another surgery, will I only be able to have traditional surgery? Are there other types of surgeries available as options?
WWP, 50~59 year old female. Ask Date: 2020/04/13
Dr. Ke Fangxu reply Surgery
The risks of complications include rectal stricture and the degree of improvement in symptoms after perforation may also be limited.
Aside from traditional surgery, as far as I know, there don't seem to be any better options available.
You may want to consult the physician who originally performed your surgery regarding further treatment options.

When considering repeat hemorrhoid surgery, particularly after an initial circular stapled procedure, it's essential to understand the potential risks and complications that may arise. Hemorrhoid surgery, while generally safe, can lead to various complications, especially if performed multiple times.
Risks of Repeat Hemorrhoid Surgery
1. Infection: Any surgical procedure carries the risk of infection. Repeat surgeries may increase this risk, particularly if the tissue has been previously manipulated or scarred.
2. Bleeding: Post-operative bleeding is a common complication. If you are already experiencing bleeding with bowel movements, the risk of further bleeding increases with another surgery.
3. Anal Stenosis: This is a narrowing of the anal canal that can occur after surgery, leading to difficulties with bowel movements and requiring further intervention.
4. Fecal Incontinence: Although rare, there is a risk of nerve damage during surgery that could lead to fecal incontinence, particularly if the surgery is extensive or if there is significant scarring from previous surgeries.
5. Chronic Pain: Some patients may experience chronic pain after hemorrhoid surgery, which can be exacerbated by repeat procedures.
6. Recurrence of Hemorrhoids: Even after surgery, there is a possibility that hemorrhoids can recur, especially if lifestyle factors contributing to hemorrhoid formation (such as constipation or prolonged sitting) are not addressed.
Options for Treatment
If you are experiencing ongoing symptoms such as bleeding, it is crucial to consult with a healthcare provider to evaluate the best course of action. Here are some potential options:
1. Conservative Management: Before considering another surgery, it may be worthwhile to explore conservative treatments. This includes dietary changes (increasing fiber intake), hydration, and the use of stool softeners to prevent straining during bowel movements.
2. Medications: Topical treatments, such as creams or ointments that contain hydrocortisone, may help reduce inflammation and discomfort. Additionally, oral medications may be prescribed to manage pain or inflammation.
3. Non-Surgical Procedures: There are several minimally invasive procedures that can be considered, such as:
- Rubber Band Ligation: This involves placing a rubber band around the base of the hemorrhoid to cut off its blood supply, causing it to shrink and fall off.
- Sclerotherapy: This involves injecting a solution into the hemorrhoid to shrink it.
- Infrared Coagulation: This technique uses infrared light to coagulate the blood vessels supplying the hemorrhoid.
4. Traditional Surgery: If surgery is deemed necessary, traditional hemorrhoidectomy may be an option. This involves the surgical removal of hemorrhoids and may be more effective for severe cases or when other treatments have failed.
Conclusion
In summary, while repeat hemorrhoid surgery can be effective, it does come with increased risks and potential complications. It is essential to have a thorough discussion with your healthcare provider about your symptoms, previous treatments, and the best approach moving forward. They can help you weigh the risks and benefits of additional surgery versus alternative treatments, ensuring that you make an informed decision that prioritizes your health and well-being.
