Scratching Issues
Hello, Doctor.
I’m not sure if it’s because I’ve been washing my hands and using alcohol frequently lately, but my skin feels fragile.
I accidentally scratched my hand on a wooden board, and now there’s a red mark.
I would like to know if I need to go get a tetanus shot again, and whether I should be concerned about infection.
Last year, when I got a puncture wound from a metal hook, my family took me to get a tetanus shot.
There wasn’t much bleeding, but when hydrogen peroxide was applied, it stung a bit.
I only applied a neomycin ointment from a year ago; I’m not sure if that’s appropriate.
Thank you.

Dr. Ye Qianyu reply Family Medicine
Hello! The tetanus vaccine is effective for 5 to 10 years after administration, so there is no need for a booster shot during that period.
Regarding wound care, based on your description, it appears to be a superficial wound, so hydrogen peroxide is not necessary.
You can clean it with saline solution, dry it, and then apply an ointment.
Thank you for your question, and if you have any issues, please visit the surgical outpatient clinic for treatment.

When it comes to minor scratches, the need for a tetanus shot largely depends on several factors, including the nature of the injury, your vaccination history, and the type of object that caused the scratch. Tetanus is a serious bacterial infection caused by Clostridium tetani, which can enter the body through wounds, particularly deep puncture wounds or those contaminated with dirt, feces, or saliva.
For minor scratches, such as the one you described from a wooden board, the risk of tetanus infection is generally low, especially if the scratch is superficial and clean. However, if the scratch is deep or if the wood was dirty, there may be a higher risk. Here are some guidelines to consider:
1. Vaccination History: If you have received a tetanus booster shot within the last 10 years, you are likely protected against tetanus. The Centers for Disease Control and Prevention (CDC) recommends that adults receive a tetanus booster every 10 years. If your last booster was more than 10 years ago, or if you are unsure of your vaccination status, it may be advisable to get a booster.
2. Type of Injury: If the scratch was caused by a clean object and is not deep, the risk of tetanus is lower. However, if the scratch was caused by a rusty or dirty object, or if it is deep, you should consult a healthcare professional about the need for a tetanus shot.
3. Signs of Infection: Monitor the scratch for signs of infection, which may include increased redness, swelling, warmth, pus, or fever. If you notice any of these symptoms, seek medical attention promptly.
4. First Aid: For minor scratches, clean the wound thoroughly with soap and water, and apply an antiseptic. Neomycin ointment, which you mentioned, is an antibiotic ointment that can help prevent infection in minor cuts and scrapes. However, it is important to use it as directed and to avoid using it on large areas of broken skin.
5. Consultation: If you are unsure whether you need a tetanus shot or if you have any concerns about the scratch, it is always best to consult with a healthcare provider. They can assess your injury and vaccination history and provide personalized recommendations.
In summary, for a minor scratch from a wooden board, if you are up to date on your tetanus vaccinations and the scratch is clean and not deep, you may not need a tetanus shot. However, if you have any doubts or if the scratch shows signs of infection, it is wise to seek medical advice. Always prioritize proper wound care and hygiene to minimize the risk of infection.
