Left lower abdominal dull pain?
Hello, doctor: I have been experiencing a dull pain in my lower left abdomen (near the waist) for the past two days.
I'm not sure if it is related to my menstrual period (which did not occur in February and March, but started on April 13), but I did not have this issue during previous menstrual cramps.
I have a history of endometriosis with chocolate cysts, so I would like to know if this could be a recurrence or if there might be another cause.

Acute or chronic lower abdominal pain can have many underlying causes, and it is recommended to seek evaluation from a specialist in an outpatient setting.

Left lower abdominal pain in women can be attributed to a variety of causes, and understanding these potential reasons is crucial for determining the appropriate course of action. Given your history of endometriosis, specifically chocolate cysts (endometriomas), it is essential to consider this in the context of your current symptoms.
Firstly, the pain you are experiencing could indeed be related to your menstrual cycle. Hormonal fluctuations during the menstrual cycle can lead to various symptoms, including cramping and discomfort in the lower abdomen. If your period was delayed for a couple of months and has just started, it is possible that your body is adjusting to the hormonal changes, which could manifest as pain.
However, since you have a history of chocolate cysts, it is also important to consider the possibility of recurrence or complications related to these cysts. Chocolate cysts are a type of ovarian cyst that can develop as a result of endometriosis. They are filled with old blood and can cause pain, especially during menstruation or ovulation. If the cysts have grown or if there is a rupture, this could lead to significant discomfort.
Other potential causes of left lower abdominal pain include:
1. Ovarian Issues: Aside from chocolate cysts, other ovarian conditions such as functional cysts, ovarian torsion, or even ovarian cancer (though less common in younger women) could present with similar symptoms.
2. Pelvic Inflammatory Disease (PID): This is an infection of the female reproductive organs that can cause pain and is often associated with sexually transmitted infections. Symptoms may include fever, unusual discharge, and pain during intercourse.
3. Gastrointestinal Issues: Conditions such as irritable bowel syndrome (IBS), diverticulitis, or constipation can also cause left-sided abdominal pain. Given that you mentioned the pain is near the waist, it could be related to bowel function.
4. Musculoskeletal Pain: Sometimes, pain in the lower abdomen can be referred pain from the muscles or ligaments in the pelvic area, especially if there has been any recent physical activity or strain.
5. Urinary Tract Issues: Conditions such as a urinary tract infection (UTI) or kidney stones can also cause pain in the lower abdomen, although they are typically accompanied by other symptoms like burning during urination or changes in urinary frequency.
Given your concerns about the recurrence of chocolate cysts and the nature of your pain, it is advisable to seek medical attention. A healthcare provider may recommend imaging studies, such as an ultrasound, to evaluate the ovaries and check for any abnormalities. Additionally, a pelvic exam may be necessary to assess for any signs of infection or other issues.
In summary, while your left lower abdominal pain could be related to your menstrual cycle, the history of chocolate cysts raises the possibility of recurrence or other complications. It is essential to consult with a healthcare professional who can perform the necessary evaluations and provide a tailored diagnosis and treatment plan. Early intervention can help manage symptoms effectively and address any underlying conditions.
