Post-circumcision care
Hello, doctor.
It has been five weeks since my circumcision.
For the first three weeks, my erections were normal, but later on, I noticed that the area around the stitches feels tighter during erections (the stitches were removed two weeks after the procedure).
Is this normal? Why is this happening? Should I frequently have erections to help loosen it, or can I apply a steroid cream? Or should I not worry about it, as it will gradually loosen over time? How long will it take to improve?
Niming, 20~29 year old female. Ask Date: 2020/04/17

Hello: This platform cannot recommend treatments; please seek medical attention.
The use of steroid creams is generally limited, and the frequency should not be excessive.
Wishing you good health.

Post-circumcision concerns, particularly regarding tightness during erections, can be a common issue that many men experience after the procedure. It is important to understand the healing process and the factors that may contribute to the sensation of tightness.
After circumcision, the area around the glans (the head of the penis) can undergo significant changes. The skin that was previously covered by the foreskin is now exposed, and the healing process can lead to some degree of scarring or tightening of the skin. This tightening can sometimes be felt during erections, especially if the skin has not fully adjusted to its new position.
In your case, experiencing tightness during erections five weeks post-surgery, especially after having normal erections for the first three weeks, may be attributed to a few factors:
1. Healing Process: The skin and tissues are still healing, and it is not uncommon for the sensation to change as the healing progresses. Scar tissue can form, which may feel tighter than the surrounding skin.
2. Skin Elasticity: The skin around the glans may take time to regain its elasticity. This can lead to a feeling of tightness during erections, as the skin may not stretch as easily as it did before the circumcision.
3. Inflammation or Sensitivity: The area may still be sensitive or slightly inflamed, which can contribute to discomfort or a feeling of tightness.
Regarding your questions about managing this tightness:
- Erections: It is generally beneficial to have regular erections as this can help the skin adapt and stretch over time. However, if you experience significant discomfort, it is advisable to avoid forcing erections.
- Topical Steroids: Applying a mild topical steroid cream may help reduce inflammation and promote skin elasticity. However, this should only be done under the guidance of a healthcare provider, as inappropriate use of steroids can lead to skin thinning or other complications.
- Timeframe for Improvement: The timeframe for improvement can vary from person to person. Many men find that the tightness diminishes over several weeks to months as the skin continues to heal and adapt. If the tightness persists beyond a few months or if you experience pain, it is important to consult with your urologist or healthcare provider.
- Follow-Up: If you have concerns about the tightness or if it is accompanied by pain, discomfort, or any other unusual symptoms, it is crucial to follow up with your healthcare provider. They can assess the situation and determine if any further intervention is necessary.
In summary, while some tightness during erections post-circumcision can be normal, it is essential to monitor the situation and seek medical advice if you have concerns. Regular follow-ups with your healthcare provider can help ensure that your healing process is on track and that any issues are addressed promptly.
