Dear Doctor, my wife (35 years old) has chocolate cysts on both ovaries, one measuring 2.9 cm and the other over 4 cm.
Since we would like to have a second child and her AMH level is only 1.33, many doctors have advised against laparoscopic surgery to avoid further decreasing her AMH level.
The recommendations have been to perform "cyst aspiration outpatient surgery + in vitro fertilization (IVF)." However, there are differing opinions among doctors regarding the timing of the cyst aspiration procedure.
Doctors A and B suggest that the aspiration surgery should be performed "at the onset of menstruation," with the goal of clearing the blood from the ovaries early in the menstrual cycle to facilitate follicle growth.
On the other hand, Doctor C believes that the aspiration surgery only addresses the symptoms and not the underlying issue, and that the cyst will likely recur 2-3 days post-surgery.
Therefore, Doctor C recommends that the aspiration procedure be performed "during the egg retrieval process."
Although we raised the opinions of Doctors A and B during our consultation with Doctor C, he still maintained that the aspiration should be done during egg retrieval.
