Ejaculation issues

Hello doctor, I am 20 years old.
My secondary sexual characteristics, such as the development of Adam's apple and body hair, are all normal.
I do not engage in masturbation, nor have I experienced nocturnal emissions, and I have not ejaculated at all.
Could it be that the lack of masturbation is causing a decrease in sperm production? If not, where are the sperm that are being produced going? I am concerned that I might not be able to ejaculate during future sexual activity.

Allk, 20~29 year old female. Ask Date: 2020/04/20

Dr. Li Xinyi reply Urology

Sperm that is not ejaculated will be absorbed by the body, so there is no need to worry.

Understanding ejaculation issues, especially among young men, can be a complex topic that intertwines physiological, psychological, and lifestyle factors. At 20 years old, it’s important to recognize that sexual development varies widely among individuals, and not having experienced ejaculation or orgasm yet is not necessarily a cause for concern.

Ejaculation and Sperm Production
Firstly, it’s crucial to understand that sperm production occurs continuously in the testes. The average male produces millions of sperm daily, regardless of whether he engages in sexual activity or masturbation. If you have not ejaculated yet, the sperm does not simply disappear; instead, it is reabsorbed by the body. This is a natural process and is quite common, especially among younger men who may not have engaged in sexual activity or masturbation.

The Role of Masturbation
Masturbation is a normal and healthy sexual activity that many young men engage in. It can help in understanding one’s body, sexual response, and ejaculation. However, the absence of masturbation does not inherently lead to a decrease in sperm production. Your body is capable of producing sperm regardless of your sexual activity. If you are concerned about your sperm count or ejaculation, it might be beneficial to consider that sexual arousal and ejaculation can also be influenced by psychological factors, such as anxiety or stress, which can affect sexual performance and desire.

Psychological Factors
Many young men experience anxiety regarding sexual performance, especially if they feel they are behind their peers in sexual experiences. This anxiety can lead to a cycle of worry that may inhibit sexual arousal and ejaculation. It’s essential to approach sexual experiences with a relaxed mindset. If you find yourself feeling anxious about the prospect of ejaculation or sexual activity, consider speaking with a healthcare professional or a counselor who specializes in sexual health. They can provide guidance and reassurance.

Physical Health Considerations
If you have concerns about your sexual health, including the ability to ejaculate or any other issues, it may be worth consulting a healthcare provider. They can perform a physical examination and possibly recommend tests to ensure that everything is functioning normally. Conditions such as hormonal imbalances, anatomical issues, or other medical concerns can sometimes affect sexual function, but these are less common in young, otherwise healthy individuals.

Lifestyle Factors
Your lifestyle can also play a significant role in sexual health. Factors such as diet, exercise, sleep, and stress management can all impact your sexual function. Maintaining a balanced diet, engaging in regular physical activity, and ensuring adequate sleep can contribute positively to your overall health and sexual well-being.

Conclusion
In summary, not having ejaculated by the age of 20 is not uncommon, and it does not necessarily indicate a problem with sperm production or sexual health. Your body is designed to manage sperm production and reabsorption naturally. If you have concerns about your sexual health or experience anxiety regarding sexual performance, consider seeking professional advice. Remember, sexual health is a normal part of life, and addressing any worries you have can lead to a more fulfilling sexual experience in the future.
