Irregular Menstrual Cycles and Pregnancy Testing Accuracy - Obstetrics and Gynecology

Pregnancy probability

Hello, Doctor: My menstrual cycle has been very irregular and chaotic.
Sometimes it comes every 28 days, other times 35 or 45 days, and in the past year, there have been instances where I didn't have a period for two months.
Even when I do have my period, it only lasts for 3-4 days.
I started having sexual intercourse with my partner in mid-March of this year (always using condoms and with external ejaculation).
The last time we had intercourse was on March 28, and my period started on March 29 and lasted for about 4 days.
I have taken pregnancy tests using morning urine and a brand of pregnancy test strips (Manshuilei) 11, 14, 18, 20, 25, and 30 days after intercourse, and all tests were negative after being left for a day.
I would like to ask the doctor two questions: 1.
Are these pregnancy test results reliable? Can I rule out the possibility of pregnancy (i.e., just irregular menstruation)? 2.
Is there a need to take another pregnancy test? If so, when would be the best time to test for accuracy? Thank you.

Niming, 20~29 year old female. Ask Date: 2020/04/29

Hello, if you take a pregnancy test 12 days after sexual intercourse and it is negative, it is unlikely that you are pregnant.
Wishing you good health.

Irregular menstrual cycles can be quite challenging for many women, especially when it comes to understanding their fertility and the accuracy of pregnancy tests. Based on your description, it seems that you have been experiencing significant irregularities in your menstrual cycle, which can complicate the process of determining whether or not you are pregnant.

1. Reliability of Pregnancy Test Results: The pregnancy tests you have taken are generally reliable, especially if they are taken correctly and at the right time. Most home pregnancy tests, including the one you mentioned (曼秀雷敦), are designed to detect the hormone hCG (human chorionic gonadotropin) in urine. This hormone is typically produced shortly after a fertilized egg attaches to the uterine lining, which usually occurs about 6 to 12 days after conception. Given that you have taken multiple tests at various intervals after your last sexual encounter on March 28, and all tests returned negative results, it is reasonable to conclude that the likelihood of pregnancy is low. However, the irregularity of your menstrual cycle can sometimes lead to delayed ovulation, which might affect the timing of hCG production and, consequently, the accuracy of the tests.

2. When to Retest for Pregnancy: If you still suspect that you might be pregnant despite the negative tests, it is advisable to wait a few days to a week after your expected period to retest. Since your cycles are irregular, it might be difficult to pinpoint when your next period is due. If you do not get your period within a week of when you expect it, taking another pregnancy test would be a good idea. Additionally, using first-morning urine can increase the accuracy of the test, as it contains the highest concentration of hCG.

Additional Considerations: Given your history of irregular cycles, it may be beneficial to consult with a healthcare provider. They can help assess your menstrual irregularities and discuss potential underlying causes, such as hormonal imbalances, polycystic ovary syndrome (PCOS), or other conditions that might affect your menstrual cycle and fertility.
Moreover, if you find that your menstrual cycles continue to be irregular or if you experience any unusual symptoms, such as severe pain or significant changes in your menstrual flow, it would be wise to seek medical advice. A healthcare provider may suggest further testing, such as blood tests to check hormone levels, or imaging studies like an ultrasound to evaluate your reproductive organs.

In summary, based on your negative pregnancy tests and the timing of your menstrual cycle, it is likely that you are not pregnant. However, if you continue to have concerns or if your menstrual irregularities persist, consulting a healthcare professional would be the best course of action. They can provide personalized advice and support tailored to your specific situation.
