Is it true that oral isotretinoin should not be used in conjunction with certain antibiotics?
Hello, pharmacist: I am currently undergoing treatment with oral isotretinoin, but recently I have had to take antibiotics due to a cold.
The antibiotics prescribed by my doctor are CURAM 1000 mg and Klarith F.C 250 mg.
The dermatologist advised me not to use tetracycline antibiotics concurrently.
Could you please tell me what type of antibiotics these two are? Are they classified as tetracyclines, and can they be used together with oral isotretinoin? Thank you.

Dr. Liao Huiling reply Pharmacology
CURAM 1000mg and Klarith F.C 250mg are both not tetracycline antibiotics and do not have any interaction issues with oral isotretinoin.

When considering the combination of oral isotretinoin (commonly referred to as Accutane) with antibiotics, it is essential to understand both the types of antibiotics prescribed and the potential interactions with isotretinoin.
In your case, you have been prescribed CURAM (which typically contains amoxicillin and clavulanic acid) and Klarith F.C (which contains clarithromycin). Neither of these antibiotics belongs to the tetracycline class. Tetracyclines, such as doxycycline and minocycline, are specifically contraindicated with isotretinoin due to the increased risk of intracranial hypertension and other side effects.
CURAM is a combination antibiotic that works by inhibiting bacterial cell wall synthesis (amoxicillin) and preventing the breakdown of amoxicillin (clavulanic acid), making it effective against a variety of bacterial infections. Clarithromycin, on the other hand, is a macrolide antibiotic that inhibits bacterial protein synthesis, making it effective against respiratory tract infections and other bacterial infections.
The combination of isotretinoin with CURAM and clarithromycin is generally considered safe. However, it is crucial to monitor for any potential side effects. Isotretinoin can cause dryness and irritation of mucous membranes, and antibiotics can sometimes exacerbate these effects. Additionally, both isotretinoin and clarithromycin can affect liver function, so it is advisable to have liver function tests monitored if you are on these medications concurrently.
It is also important to note that isotretinoin can increase sensitivity to sunlight, and while antibiotics like clarithromycin do not typically have this effect, it is wise to take precautions against sun exposure while on these medications.
In summary, CURAM and clarithromycin are not tetracycline antibiotics and can be safely used with oral isotretinoin. However, always consult with your healthcare provider before starting any new medications or if you have concerns about potential interactions. They can provide personalized advice based on your medical history and current treatment plan.
Additionally, if you experience any unusual symptoms or side effects while taking these medications together, it is essential to contact your healthcare provider promptly. Regular follow-ups and communication with your healthcare team are vital to ensure the effectiveness of your treatment while minimizing risks.
