Exploring Steroid Ointments for Phimosis: Alternatives to Circumcision - Urology

Dilation of the preputial orifice?

Hello Doctor: Currently, my situation is that the foreskin can retract without pain when not erect, and it can retract behind the coronal sulcus when erect, but it feels tight.
Before high school, I was unable to retract the foreskin at all, but after making an effort to gradually retract it, I have made progress and am now happy to be able to clean the smegma daily.
Later, I saw online that some people successfully retracted their foreskin completely without pressure using a steroid ointment, which has opened a glimmer of hope for me.
I would like to ask if using a steroid ointment would be effective for my condition.
I am not very fond of seeing doctors, and in cases like this, doctors often strongly recommend circumcision.
Since I am more introverted, I find it hard to resist such strong recommendations from doctors, so I am here to inquire if there are alternative methods to address this issue.

Hello: It is effective but may be slow, and there may be some changes in the skin, which can vary from person to person.
You also understand that some people are very concerned about using steroids, especially with long-term use.
Wishing you good health.

Phimosis, the condition where the foreskin cannot be easily retracted over the glans of the penis, can be a source of discomfort and concern for many men. Your experience of being able to retract the foreskin when not erect, but facing tightness during an erection, is common among those with phimosis. The gradual improvement you’ve noted through progressive stretching is a positive sign, as many individuals can achieve better foreskin mobility with consistent effort.

The use of topical corticosteroids is indeed a recognized alternative to circumcision for managing phimosis. Corticosteroid ointments, such as betamethasone or hydrocortisone, can help soften and thin the foreskin, making it easier to retract. The mechanism behind this is that corticosteroids reduce inflammation and promote skin elasticity, which can facilitate the stretching of the foreskin over time. Studies have shown that applying these ointments in conjunction with gentle stretching exercises can lead to significant improvement in foreskin retraction in many cases.

For your situation, where you have already made progress with manual stretching, incorporating a topical steroid could potentially enhance your results. Typically, the recommended approach involves applying a thin layer of the steroid ointment to the foreskin and glans once or twice daily for a few weeks, followed by continued stretching exercises. This method has been shown to be effective in many patients, allowing them to avoid surgical intervention.

However, it is crucial to approach this treatment under the guidance of a healthcare professional. While you may feel hesitant to consult a doctor due to past experiences or a preference for avoiding medical visits, a urologist or dermatologist can provide tailored advice and monitor your progress. They can also help ensure that the steroid is used safely and effectively, minimizing the risk of side effects such as skin thinning or irritation.

If you are concerned about the pressure to undergo circumcision, it may be helpful to express your feelings to your healthcare provider. Many doctors understand that patients have different comfort levels with surgical procedures and are willing to explore conservative management options first. Open communication can lead to a more collaborative approach to your care.

In summary, using steroid ointments for phimosis can be a viable alternative to circumcision, especially in cases like yours where some retraction is possible. It is advisable to seek professional guidance to ensure the best outcomes and to discuss your concerns about treatment options. With the right approach, you may find a solution that aligns with your preferences and improves your condition without the need for surgery.
