Do you have issues with erectile dysfunction?

Hello, Doctor.
In recent years, I have been masturbating about once a day for a long time.
I noticed that in mid-May, during one of my masturbation sessions while watching videos, I experienced an erection, but it was only about 70% firm.
If I got distracted for even a moment, it would go soft again.
I found that I could only maintain arousal when watching certain scenes that I found more stimulating; the earlier foreplay scenes barely kept me hard, and they would also go soft with any distraction.
This issue started occurring only in mid-May; I had not experienced anything like this before.
I have always been quite sensitive, and I usually ejaculate easily.
Since mid-May, I have only experienced morning erections once.
I might have had nocturnal erections without realizing it, but I distinctly remember one morning erection since mid-May.
I currently have no sexual experience and have only engaged in self-stimulation.
I am 28 years old and have gained some weight due to frequently staying up late for work (though I had not faced such issues before).
My height is 183 cm and my weight is 88 kg.
I have developed a habit of going to bed after 1 AM, which started when I was working late into the night.
In 2018, I had a health check-up that revealed issues related to obesity, such as high weight, abnormal waist circumference, high BMI, and high cholesterol (placing me in a high-risk group for metabolic syndrome).
However, I currently have no diseases, and I do not exercise at all, not even jogging.
My diet consists mainly of eating out and only consuming foods I like.
I would like to know if this situation could be considered erectile dysfunction, or if it is difficult to say and I should first adjust my lifestyle, diet, and sleep.
If adjustments do not lead to improvement, should I seek medical attention? How long does it usually take? I am concerned that it may be too late to seek help.
During the adjustment period, how long should I wait before seeking medical attention if there is still no improvement? Thank you, Doctor.

Hello: A lack of sexual activity cannot be classified as erectile dysfunction, but your symptoms indicate that your body has undergone some changes.
It is important to exercise, get enough sleep, maintain a low-fat diet, and drink plenty of water.
Reducing the frequency of DIY activities may lead to improvement; however, it is difficult to predict the extent of recovery.
Wishing you good health.

Erectile dysfunction (ED) is a common condition that can affect men of all ages, but it is particularly prevalent among those over 40. However, younger men, like yourself at 28, can also experience ED due to various factors. Your description indicates that you have been experiencing difficulties with maintaining an erection, particularly during masturbation, and that this issue has arisen relatively recently.

Understanding Your Symptoms
1. Erection Quality: You mentioned that your erections are only about 70% firm and that they tend to soften if you lose focus. This could indicate psychological factors at play, such as anxiety or stress, which can significantly impact sexual performance. The fact that you require specific stimuli to maintain an erection suggests that your arousal may be influenced by psychological rather than purely physiological factors.

2. Morning Erections: The infrequency of morning erections can also be a sign of erectile dysfunction. Morning erections are a natural occurrence in healthy men and are often an indicator of good blood flow and hormonal levels. The fact that you have only experienced this once since mid-May could suggest a change in your physiological state.

3. Lifestyle Factors: Your lifestyle choices, including late-night work habits, lack of exercise, and poor diet, can contribute to ED. Being overweight and having high cholesterol can affect blood circulation, which is crucial for achieving and maintaining an erection. Additionally, sleep deprivation can lead to hormonal imbalances that may affect libido and erectile function.

When to Seek Help
It's essential to consider seeking medical advice if your symptoms persist. Here are some guidelines:
- Duration of Symptoms: If you have been experiencing these symptoms for more than a few weeks, it would be wise to consult a healthcare professional. Persistent ED can be a sign of underlying health issues that need to be addressed.

- Impact on Quality of Life: If your erectile difficulties are causing you distress or affecting your self-esteem, relationships, or overall quality of life, it is important to seek help sooner rather than later.

- Lifestyle Changes: Before seeking medical intervention, consider making lifestyle changes. Improving your diet, increasing physical activity, and establishing a regular sleep schedule can have a positive impact on your overall health and may improve your erectile function.
- Timeframe for Improvement: If you make significant lifestyle changes and do not see any improvement within 4 to 6 weeks, it would be advisable to consult a healthcare provider. They can conduct a thorough evaluation, which may include blood tests to check hormone levels, cholesterol, and blood sugar, as well as a physical examination.

Potential Causes of ED
ED can be caused by a variety of factors, including:
- Psychological Factors: Stress, anxiety, and depression can all contribute to erectile dysfunction. Performance anxiety, in particular, can create a cycle of anxiety and erectile difficulties.

- Physical Factors: Conditions such as obesity, diabetes, high blood pressure, and cardiovascular disease can affect blood flow and nerve function, leading to ED.

- Hormonal Imbalances: Low testosterone levels can also contribute to erectile dysfunction.

- Medications: Certain medications can have side effects that include erectile dysfunction.

Conclusion
In summary, while your symptoms may not definitively indicate erectile dysfunction, they do warrant attention. Lifestyle changes can be beneficial, but if you do not see improvement, it is essential to consult a healthcare professional. They can provide a comprehensive evaluation and discuss potential treatment options, which may include lifestyle modifications, therapy for psychological factors, or medications. Remember, seeking help is a proactive step towards better health and well-being.
