Epididymitis

Hello Doctor: I am studying in the Netherlands.
Two weeks ago, I experienced swelling in my left scrotum, which subsided after three days.
However, I then noticed a small hard lump.
I visited my family doctor, who performed a physical examination and diagnosed it as epididymitis, prescribing a three-day course of antibiotics.
During my follow-up visit, an ultrasound was performed, which showed that my testicles were normal, and I am no longer in pain, but the small hard lump remains.
I would like to ask if a three-day course of antibiotics is sufficient? Besides maintaining a regular schedule and drinking plenty of water, what else can I do? Thank you!

Niming, 20~29 year old female. Ask Date: 2020/05/26

Dr. Cai Zhuorong reply Urology

The recommended antibiotic treatment course for epididymitis is 10 days.
If it recurs, further evaluation should be conducted.

Epididymitis is an inflammation of the epididymis, a tube located at the back of the testicle that stores and carries sperm. It can be caused by various factors, including bacterial infections, sexually transmitted infections, or even non-infectious causes. Your experience of swelling in the left scrotum followed by the development of a hard lump is consistent with this condition.
Regarding your treatment, a three-day course of antibiotics may be sufficient for mild cases of epididymitis, especially if the symptoms have resolved and the ultrasound showed no abnormalities in the testicle itself. However, the duration of antibiotic therapy can vary based on the underlying cause of the infection. For bacterial epididymitis, especially if it is caused by sexually transmitted infections, a longer course of antibiotics (typically 10 to 14 days) is often recommended to ensure complete resolution of the infection. Since you mentioned that you are no longer in pain, it is a positive sign, but the presence of the hard lump may warrant further evaluation.

In terms of recovery and management, here are some tips that may help:
1. Rest and Activity Modification: It is important to avoid activities that may exacerbate the condition. Gentle activities are generally fine, but avoid heavy lifting or strenuous exercise until you are fully recovered.

2. Hydration: Staying well-hydrated is crucial. Drinking plenty of water can help flush out any remaining bacteria and support your overall health.

3. Pain Management: If you experience any discomfort, over-the-counter pain relievers such as ibuprofen or acetaminophen can be helpful. Always follow the recommended dosages and consult with a healthcare provider if you have any concerns.

4. Supportive Garments: Wearing supportive underwear can help alleviate discomfort and provide support to the scrotal area.

5. Follow-Up Care: Since you still have a hard lump, it is advisable to follow up with your healthcare provider. They may recommend further imaging or examination to ensure that there are no complications, such as a persistent infection or a cyst.

6. Monitor Symptoms: Keep an eye on any changes in your symptoms. If you notice increased swelling, pain, fever, or any other concerning symptoms, seek medical attention promptly.

7. Sexual Activity: If your epididymitis was caused by a sexually transmitted infection, it is important to abstain from sexual activity until you have completed your antibiotic course and have been cleared by your healthcare provider.

8. Education and Prevention: Understanding the causes of epididymitis can help in preventing future occurrences. Practicing safe sex, maintaining good hygiene, and seeking prompt treatment for urinary tract infections can reduce your risk.

In conclusion, while a three-day course of antibiotics may be adequate for some cases, it is essential to follow up with your healthcare provider regarding the hard lump and ensure that you have fully recovered. Taking care of your overall health, staying hydrated, and monitoring your symptoms will aid in your recovery. If you have any further questions or concerns, do not hesitate to reach out to a healthcare professional.
