Post-thyroid lobectomy neck?
I would like to ask the doctor, I underwent a total right thyroid lobectomy with unilateral neck lymphadenectomy in March of this year.
Upon discharge, I experienced numbness in the right side of my neck, but it has gradually improved.
The swelling above the incision site is also slowly decreasing, but recently I have noticed some mild swelling in the right side of my neck and above the incision site.
It is not significantly swollen, but I do feel a more pronounced pulling sensation in my neck when swallowing.
I would like to know if this is a normal phenomenon of lymphedema? What are the possible ways to manage lymphedema? Thank you.
Jiajia, 30~39 year old female. Ask Date: 2020/05/29
Dr. Li Jian reply Internal Medicine
It is recommended to discuss postoperative recovery matters with the original surgeon to address how to alleviate and improve the situation.

More Info
After a total thyroidectomy, especially when accompanied by the removal of lymph nodes, it is not uncommon to experience various symptoms in the neck area, including numbness, swelling, and a sensation of tightness or pulling during swallowing. These symptoms can be attributed to several factors, including surgical trauma, inflammation, and the potential development of lymphedema.
Understanding Post-Thyroidectomy Symptoms
1. Numbness and Tingling: The numbness you are experiencing on the right side of your neck is likely due to nerve irritation or damage during surgery. The vagus nerve and recurrent laryngeal nerve, which are crucial for swallowing and voice, can be affected during thyroid surgery. While some numbness may persist, many patients experience gradual recovery over time.
2. Swelling: The swelling above your incision site and in the neck can be a result of postoperative inflammation. This is a normal response to surgery as the body heals. However, if the swelling persists or worsens, it may indicate the onset of lymphedema, which is a condition characterized by the accumulation of lymph fluid due to disrupted lymphatic drainage.
3. Lymphedema: This condition can occur when lymph nodes are removed or damaged, leading to fluid buildup. Symptoms of lymphedema include swelling, a feeling of heaviness, and sometimes discomfort in the affected area. In your case, the slight swelling and pulling sensation during swallowing could suggest early signs of lymphedema.
Management of Lymphedema
If you suspect that you are developing lymphedema, it is essential to address it promptly to prevent further complications. Here are some management strategies:
1. Compression Garments: Wearing a compression garment can help reduce swelling by promoting lymphatic drainage. These garments are specifically designed for lymphedema management and should be fitted by a professional.
2. Manual Lymphatic Drainage (MLD): This is a specialized massage technique performed by trained therapists to encourage lymph fluid movement and reduce swelling. MLD can be beneficial in the early stages of lymphedema.
3. Exercise: Gentle exercises can help stimulate lymphatic flow. Activities such as walking, swimming, or specific lymphedema exercises can be beneficial. However, it is crucial to avoid heavy lifting or strenuous activities that may exacerbate swelling.
4. Skin Care: Keeping the skin clean and moisturized is vital to prevent infections, which can worsen lymphedema. Be cautious with any cuts or abrasions in the affected area.
5. Elevation: Elevating the affected arm or leg can help reduce swelling. When resting, try to keep the affected area elevated above the level of your heart.
6. Consultation with Specialists: If symptoms persist or worsen, it is advisable to consult with a lymphedema specialist or a physical therapist experienced in managing post-surgical lymphedema. They can provide tailored treatment plans and monitor your progress.
Conclusion
In summary, the symptoms you are experiencing post-thyroidectomy, including numbness and swelling, can be part of the normal healing process. However, the development of lymphedema is a possibility that should be monitored closely. Implementing the management strategies mentioned above can help alleviate symptoms and improve your quality of life. Always keep an open line of communication with your healthcare provider regarding any changes in your symptoms, as they can provide the most personalized and effective care.
