Ovulation Issues: Can Delayed Release Affect Egg Quality? - Obstetrics and Gynecology

Ovulation issues

Hello Dr.
Tsao,
I would like to ask about my menstrual cycle, which is approximately 35-40 days long.
I understand that ovulation typically occurs around 14 days before menstruation.
If I calculate backwards from a 35-day cycle, ovulation would be on day 22.
However, during an ultrasound on day 18, it was observed that the follicle was already 3 cm but had not yet released the egg.
Does this mean that by day 22, if the egg is released, it may have aged and therefore could be unable to be fertilized?

Chloe, 30~39 year old female. Ask Date: 2020/05/30

Dr. Cao Guotao reply Obstetrics and Gynecology

Please follow the doctor's instructions.
Wishing you good health.

Understanding ovulation and its implications for egg quality is crucial for women who are planning to conceive or are considering options like egg freezing. Your question touches on several important aspects of the menstrual cycle, ovulation timing, and the potential impact on egg quality.

Firstly, it's essential to clarify the ovulation process. Typically, ovulation occurs approximately 14 days before the start of your next menstrual period. In a 35-day cycle, ovulation would generally occur around day 21. However, variations can occur, and factors such as hormonal imbalances, stress, and lifestyle can influence the timing of ovulation.

When you mention that an ultrasound on day 18 showed a 3 cm follicle that had not yet released an egg, it indicates that your body was preparing for ovulation. A follicle of this size is typically mature and ready to release an egg. If ovulation occurs after this point, the egg's viability is a critical factor to consider. Generally, an egg is viable for fertilization for about 12 to 24 hours after ovulation. If ovulation occurs later than expected, the egg may not be as healthy or viable, especially if it is released much later than the typical ovulation window.

Regarding the concern about the egg aging, it is important to note that while the quality of an egg can decline as it remains in the follicle, the primary factor affecting egg quality is the age of the woman. As women age, the quality and quantity of their eggs decrease, which can lead to challenges in conception and increased risks of chromosomal abnormalities. However, if you are in your reproductive years, a single cycle's delay in ovulation may not significantly impact the egg's quality, provided that it is released within a reasonable timeframe.

If you are considering freezing your eggs, timing is crucial. It is generally recommended to undergo egg retrieval when the follicles are mature, which is typically indicated by their size (around 18-20 mm) and hormonal levels (such as estradiol). If you are experiencing irregular cycles or delayed ovulation, it may be beneficial to consult with a fertility specialist. They can provide insights into your specific situation, perform necessary tests, and guide you on the best timing for egg retrieval to maximize the chances of obtaining high-quality eggs.

In conclusion, while delayed ovulation can potentially impact egg quality, it is essential to consider the broader context of your reproductive health. If you have concerns about your menstrual cycle, ovulation timing, or egg quality, seeking advice from a healthcare provider specializing in reproductive health can provide personalized guidance and support. They can help you understand your cycle better and make informed decisions regarding conception or egg freezing.
