After the recovery from corneal ulcer, the vision remains unclear?

Dear Dr.
Gao,
In mid-May of this year, I sought medical attention due to a foreign body sensation in my eye.
The clinic diagnosed me with a corneal ulcer and referred me to a larger medical facility.
After visiting the larger hospital, the doctors conducted several tests.
Upon examining my eye, they confirmed that I indeed had a corneal ulcer.
I felt some improvement, and they suggested that the eye drops prescribed by the initial clinic might have been effective.
They conducted further tests, which showed no signs of bacterial, viral, or fungal infection.
However, after using antibiotic eye drops, I noticed significant improvement in my eye condition.
I had two follow-up appointments, and the doctor mentioned that after finishing this bottle of medication, I would not need to return for further visits.
At that time, my eye was no longer red and showed no symptoms.
Recently, however, I have noticed a slight decrease in vision in the eye affected by the corneal ulcer.
It is not severe, but it feels like my myopia has worsened, and distant objects appear somewhat blurry and slightly distorted.
I would like to ask Dr.
Gao if this is a normal reaction following a corneal ulcer, and if there are any specific concerns I should be aware of.
Thank you for your response.

Hello Mr.
Jan: After the healing of a corneal ulcer, scar tissue may form on the cornea.
If the scar obstructs the visual axis, it can affect vision.
Your condition seems to be a result of the corneal ulcer.
I recommend discussing this situation with your attending physician to see if there are any other issues; if not, you can rest assured.
The ophthalmology department at Puzi Hospital in Taiwan is available: Monday, Tuesday, Thursday, and Friday from 8:00 AM to 12:00 PM, and Wednesday from 2:00 PM to 4:30 PM.
Please note that online responses are for reference only; it is necessary to seek medical attention at a healthcare facility if needed.
I hope to have the opportunity to serve you.
Gao Pilin

Hello! Thank you for reaching out with your concerns regarding your vision after recovering from a corneal ulcer. It's understandable to feel anxious about changes in your eyesight, especially after experiencing a significant eye condition like a corneal ulcer. Let’s address your questions and provide some insights based on your situation.

Understanding Vision Changes After Corneal Ulcer Recovery
1. Normal Recovery Process: After a corneal ulcer, it is not uncommon to experience some residual vision changes. The cornea is a delicate structure, and even after the ulcer heals, the corneal tissue may not return to its original state immediately. This can lead to symptoms such as blurred vision, sensitivity to light, and even slight distortion in how you perceive objects. These changes can be attributed to scarring or irregularities in the corneal surface that may develop during the healing process.

2. Vision Fluctuations: The sensation of your vision feeling like it has worsened, akin to an increase in nearsightedness, can occur due to several factors. The cornea's curvature may have changed due to the ulcer or subsequent healing, leading to refractive errors. Additionally, if there is any residual swelling or irregularity in the cornea, it can cause light to scatter, resulting in halos or blurriness when looking at distant objects.

3. When to Seek Further Evaluation: While some changes in vision can be expected, it is crucial to monitor your symptoms. If you notice a significant decline in your vision, persistent discomfort, or any new symptoms such as increased redness, pain, or discharge, you should return to your eye care provider for a thorough examination. They may perform tests to assess the cornea's surface and check for any complications such as scarring or irregular astigmatism.

4. Management and Treatment Options: Depending on the findings from your follow-up examination, your doctor may recommend several approaches to improve your vision:
- Prescription Glasses or Contact Lenses: If the cornea has healed but left some irregularities, corrective lenses may help improve your vision.

- Artificial Tears: These can help keep the eye lubricated and may alleviate some discomfort associated with dryness or irritation.

- Surgical Options: In cases where scarring significantly affects vision, procedures like corneal cross-linking or even corneal transplantation may be considered, but these are typically reserved for more severe cases.

5. Ongoing Care: It is essential to maintain regular follow-ups with your eye care provider, especially after a significant eye condition. They can monitor your healing process and make timely interventions if necessary. Additionally, practicing good eye hygiene and protecting your eyes from irritants can support your recovery.

6. Lifestyle Considerations: If you work long hours on the computer or engage in activities that strain your eyes, ensure you take regular breaks and practice the 20-20-20 rule: every 20 minutes, look at something 20 feet away for at least 20 seconds. This can help reduce eye strain and discomfort.

Conclusion
In summary, experiencing some vision changes after a corneal ulcer is not uncommon, but it is essential to stay vigilant and communicate with your eye care provider about any concerns. Regular check-ups will help ensure that your eyes are healing properly and that any necessary adjustments to your treatment plan can be made. Remember, your eye health is paramount, and addressing any issues promptly can lead to better outcomes. If you have further questions or need clarification, don’t hesitate to reach out to your healthcare provider. Thank you for your inquiry, and I wish you a smooth recovery and improved vision!
