Postoperative constipation after hemorrhoid surgery?

Hello: It has been 3 months since my fourth-degree hemorrhoid surgery, and recently, if I don't have a bowel movement for just 2 days, I find it difficult to pass stool.
Even after taking stool softeners, I experience abdominal cramping and still feel a blockage at the anal opening, making it hard to open up.
My stools have also become as thin as a finger.
Is this a transitional phase? Can I take stool softeners daily as a preventive measure? Would warm water baths or suppositories help improve the situation? Thank you!

Miss Yao, 30~39 year old female. Ask Date: 2020/06/03

Dr. Ke Fangxu reply Surgery

After hemorrhoid surgery, it is important to avoid constipation.
If taking stimulant laxatives causes abdominal cramping, alternative types of laxatives can be considered.
The presence of thin stools may be related to prolonged straining during bowel movements.
It is necessary to consult a physician for a digital rectal examination to determine whether anal stenosis is present.

After undergoing hemorrhoid surgery, it's not uncommon to experience various bowel issues, including constipation and changes in stool consistency. Given that you had a fourth-degree hemorrhoid surgery three months ago, your symptoms could be attributed to several factors, including the healing process, dietary habits, and the effects of medication.

Firstly, it's important to understand that after any surgical procedure involving the rectal area, such as hemorrhoid surgery, the body goes through a healing phase. During this time, the muscles and tissues around the anus may be sensitive and not function as efficiently as they did before. This can lead to difficulties in bowel movements, including the sensation of blockage and the passage of thin stools, which you described as resembling a finger. This could indicate that the anal canal is still adjusting post-surgery, and the surrounding tissues may be inflamed or sensitive.

Constipation can be particularly problematic after such surgeries. The pain associated with bowel movements may lead to a fear of straining, which can further exacerbate constipation. It's crucial to manage this effectively to avoid complications such as fecal impaction or the recurrence of hemorrhoids.
Using stool softeners can be beneficial, and it is generally safe to take them daily, especially if they help you maintain regular bowel movements without straining. However, it’s essential to consult your healthcare provider before making any changes to your medication regimen. They may recommend specific types of stool softeners or laxatives that are safe for your situation.

In addition to medication, dietary changes can significantly impact bowel health. Increasing your fiber intake by consuming more fruits, vegetables, whole grains, and legumes can help bulk up your stool and promote regularity. Staying well-hydrated is equally important, as adequate fluid intake can prevent stools from becoming hard and difficult to pass.

Warm sitz baths can also provide relief and promote relaxation of the anal muscles, which may help ease the passage of stool. If you find that you are still experiencing significant discomfort or difficulty, using a glycerin suppository or a mild laxative occasionally may provide additional relief, but again, this should be discussed with your doctor.

It's also worth noting that if your symptoms persist or worsen, or if you notice any new symptoms such as severe pain, bleeding, or changes in bowel habits, you should seek medical attention. These could be signs of complications such as anal fissures, strictures, or other issues that may require further evaluation and treatment.

In summary, managing constipation and bowel issues post-hemorrhoid surgery involves a combination of medication, dietary adjustments, and possibly lifestyle changes. Regular follow-ups with your healthcare provider are essential to monitor your recovery and make any necessary adjustments to your treatment plan. Remember, healing takes time, and it’s important to be patient with your body as it recovers from surgery.
