Bleeding from the extraction site of a horizontal wisdom tooth can occur two to three days after the stitches are removed?
On May 7, I had my lower left wisdom tooth extracted.
It has been over a month since the stitches were removed, but the wound tends to bleed every three days or so.
I returned to the hospital for a check-up, and the doctor said that my wound is healing well, but I might have a bit of debris causing inflammation.
I’ve been careful to chew only on the other side, brush my teeth after every meal, use a syringe to rinse the wound with mouthwash, and clean the nearby teeth with a water flosser.
Sometimes, I even use dental floss.
The doctor provided me with a syringe with a needle to irrigate the wound, and I have been following those instructions.
However, I still experience slight bleeding every two to three days, which lasts for about an hour at most, and it is quite distressing.
I searched online but couldn’t find anyone sharing similar experiences.
I would like to ask the doctor if I should continue using dental floss or the water flosser? What could be other causes of the bleeding besides inflammation? Can I take anti-inflammatory medication? The doctor did not prescribe any for me.
Additionally, I’m not sure if it’s related, but the molar near the wound sometimes makes a cracking sound when I apply pressure, which started right after the extraction.
It has improved somewhat, and the sound has become less frequent.
During my follow-up, the doctor took a full-mouth X-ray and didn’t find anything unusual.
I noticed that others with similar cases mentioned that such sounds occur during orthodontic treatment.

More Info
After a wisdom tooth extraction, persistent bleeding can be a concerning issue, especially if it continues for weeks after the procedure. In your case, it has been a month since the extraction of your lower left horizontal wisdom tooth, and you are experiencing intermittent bleeding every few days. This situation can be frustrating and may lead to anxiety about the healing process.
Firstly, it's important to understand that some bleeding after a tooth extraction can be normal, particularly if the area is still healing. However, if the bleeding is recurrent and occurs frequently, it may indicate an underlying issue. The fact that your dentist has assessed the healing and indicated that it is progressing well is a positive sign, but it does not eliminate the need for further investigation if the bleeding persists.
Possible Causes of Persistent Bleeding
1. Inflammation or Infection: As you mentioned, your dentist suggested that there might be some debris causing inflammation. This is a common issue after extractions, especially if food particles get trapped in the extraction site. Inflammation can lead to increased blood flow to the area, which may result in bleeding.
2. Blood Clot Displacement: After an extraction, a blood clot forms in the socket to protect the underlying bone and nerves. If this clot is dislodged or dissolves prematurely, it can lead to bleeding. This is often referred to as "dry socket," which can be quite painful and may require treatment.
3. Underlying Medical Conditions: Certain medical conditions, such as clotting disorders or the use of anticoagulant medications, can affect the body's ability to stop bleeding. If you have any underlying health issues or are taking medications that affect blood clotting, this could be a contributing factor.
4. Tissue Trauma: If you are using dental tools like a syringe or water flosser too aggressively, you might be irritating the tissue around the extraction site, leading to bleeding. It's essential to be gentle when cleaning the area.
Recommendations for Management
1. Continue Gentle Oral Hygiene: It is crucial to maintain good oral hygiene, but be cautious with the use of dental tools. If you are using a water flosser or syringe, ensure that you are not applying too much pressure. Gentle rinsing with warm salt water can help keep the area clean without causing trauma.
2. Avoid Irritants: Try to avoid hard or crunchy foods that could irritate the extraction site. Stick to softer foods that require less chewing.
3. Monitor for Signs of Infection: Keep an eye out for symptoms such as increased pain, swelling, or pus discharge, which could indicate an infection. If you notice any of these signs, it is essential to contact your dentist promptly.
4. Consult Your Dentist: Since the bleeding persists, it would be wise to return to your dentist for a follow-up. They may want to examine the site again, possibly perform imaging studies, or even refer you to a specialist if necessary.
5. Medication: If your dentist did not prescribe anti-inflammatory medication, you might consider discussing this with them. Over-the-counter options like ibuprofen can help reduce inflammation and discomfort, but always consult your dentist before starting any new medication.
6. Avoid Using Dental Floss Near the Site: Until the area is fully healed, it might be best to avoid using dental floss near the extraction site to prevent any irritation or trauma.
Conclusion
Persistent bleeding after a wisdom tooth extraction can be concerning, but with careful management and follow-up with your dentist, you can address the issue effectively. It’s essential to maintain good oral hygiene while being gentle with the extraction site. If the bleeding continues or worsens, do not hesitate to seek professional advice. Your dentist can provide the best guidance tailored to your specific situation and ensure that your healing process is on track.
