Unilateral nasal congestion?

Hello Doctor, I have been experiencing persistent unilateral nasal congestion without any allergy symptoms.
I only have nasal congestion on one side; when I change my sleeping position, the congestion shifts accordingly—left side when lying on my left, right side when lying on my right.
Sometimes it gets so severe that I suddenly can't breathe in my sleep, and I occasionally wake up with unilateral ear fullness, which doesn't improve even when I swallow.
Recently, I visited a nearby large hospital and saw an ENT specialist who performed an endoscopy.
The doctor mentioned that I have a slight deviation of the nasal septum and some swelling in the nasal turbinates (I can't recall the exact wording).
Otherwise, there were no significant issues, and I was prescribed allergy medications (Desloratadine and Fluticasone nasal spray).
I took them for fourteen days with no improvement in my unilateral nasal congestion.
Upon returning for a follow-up, the doctor prescribed me Mometasone, but it was still ineffective.
The doctor mentioned that if there is no improvement after two months of medication, I may need to undergo surgery to remove the turbinates or laser treatment.

What concerns me is whether an endoscopy can truly rule out nasopharyngeal cancer.
I haven't been sleeping well these past few days, and I keep telling myself that if the tests show nothing, then there is nothing to worry about.
However, despite taking and using the medications, I still see no improvement, and I am starting to worry about the possibility of nasopharyngeal cancer that might not have been detected.
My symptoms are quite similar, including ear fullness and unilateral nasal congestion.
I really hope I am not wasting medical resources by going to the hospital, but I am becoming anxious and developing health anxiety.
I would appreciate your insights.
Thank you.

Nasal endoscopy can visualize the majority of nasopharyngeal cancers, but a small portion may require a CT scan for assessment.
It is recommended that you visit an otolaryngology clinic for further evaluation by a physician.
Wishing you good health.

Unilateral nasal congestion can be a frustrating and concerning symptom, especially when it disrupts your sleep and daily activities. Based on your description, it seems that you have been experiencing persistent nasal congestion on one side, which worsens depending on your sleeping position. You also mentioned experiencing ear fullness and difficulty swallowing, which can be associated with nasal congestion.

The causes of unilateral nasal congestion can vary widely, and it’s essential to consider several potential factors. The most common causes include:
1. Nasal Septum Deviation: As your doctor noted, a slight deviation of the nasal septum can lead to obstruction on one side. This condition is quite common and can be exacerbated by other factors, such as nasal swelling or inflammation.

2. Nasal Polyps: These are benign growths in the nasal passages that can cause obstruction and are often associated with chronic inflammation, such as that seen in allergic rhinitis or chronic sinusitis.

3. Chronic Sinusitis: This condition can cause persistent nasal congestion and may lead to complications such as ear fullness due to Eustachian tube dysfunction.

4. Allergic Rhinitis: Although you mentioned not having typical allergy symptoms, it’s possible to have localized allergic reactions that could cause unilateral symptoms.

5. Infections: Sinus infections can lead to unilateral nasal congestion, especially if they are chronic or recurrent.

6. Tumors: While rare, tumors in the nasal cavity or nasopharynx can present with unilateral nasal obstruction and other symptoms, including ear fullness.

Regarding your concerns about nasopharyngeal cancer, it’s understandable to feel anxious, especially when experiencing symptoms that overlap with more serious conditions. However, it’s important to note that the likelihood of cancer is relatively low, especially in younger individuals without other risk factors. The use of nasal endoscopy is a valuable diagnostic tool that allows doctors to visualize the nasal passages and nasopharynx directly. While it can identify many abnormalities, it may not always detect very early or subtle lesions. If your doctor has assessed your nasal passages and found no concerning masses, that is a reassuring sign.

Given that you have tried multiple medications without relief, it may be worth discussing further options with your ENT specialist. Surgical interventions, such as turbinate reduction or endoscopic sinus surgery, can be effective for patients who do not respond to medical management. These procedures aim to improve airflow and reduce congestion by addressing the anatomical causes of your symptoms.

In the meantime, consider the following supportive measures:
- Saline Nasal Irrigation: This can help clear mucus and allergens from your nasal passages and may provide temporary relief from congestion.

- Humidifiers: Using a humidifier in your bedroom can help keep the nasal passages moist, which may alleviate some congestion.

- Avoid Irritants: Stay away from smoke, strong odors, and allergens that could exacerbate your symptoms.

Lastly, if your symptoms persist or worsen, or if you develop new symptoms such as significant pain, fever, or changes in vision, it is crucial to seek further evaluation. Your health and peace of mind are paramount, and continuing to communicate openly with your healthcare provider is essential in managing your condition effectively.
